Description Of The Task And Detailed Requirements Related To The Implementation Of The Subject Of The Order: 1) Terms Of Service: A) Service Completion Date: From The Date Of Conclusion Of The Contract To May 31, 2029; B) Monthly Commitment: Maximum 42 Hours Per Month; C) Permanent Support For People Implementing The Project; D) The Project Participants Are A Group Of 25 Children And Young People - Children Of Residents Of The Łazy Commune From Families At Risk Of Social Exclusion Or At Risk Of Poverty. 2) Detailed Scope Of Responsibilities Of The Caretaker/Educator Of A Community Center: A) Providing Support For Project Participants, In Accordance With The Identified Needs; B) Learning About The Educational Needs Of The Charges; C) Organizing And Conducting Monthly Workshop Groups; D) Organizing Integration Events For Participants; E) Motivating Project Participants To Make Progress In Improving The Skills Acquired During Classes And Improving Diagnosed Disorders, As Well As Compensating For Educational Deficiencies In The Course Of Systematic Exercises, Introducing Them To Creative Activity And Stimulating The Proper Development Of Pupils; F) Encouraging Project Participants To Systematically Work To Improve Disturbed Functions Using Various Methods Of Therapy And Forms Of Individual And Group Classes;G) Keeping Attendance Lists Of Participants During Classes;H) Active Integration Of The Participants Of The Community Center - No More Than 25 People;I) Strengthening The Participants Sense Of Belonging To The Local Community;J) Cooperation With The Project Staff In Order To Implement The Intended Activities And Achieve The Planned Project Goals;K) Cooperation With Parents (Legal Guardians) And Encouraging Them To Cooperate In Solving The Childs Problems And To Participate In The Current Life Of The Community Center;L) Providing Teaching Aids Or Using Teaching Aids Located On The Equipment Of The Facility And Development Of Materials Necessary To Implement The Subject Of The Order; M) Responsibility For The Entrusted Property; N) Participation In The Educational, Cultural And Recreational Activities Of The Community Center; O) Participation In Integration Trips, Stimulating The Activity Of Participants; P) Constant Cooperation With The Project Leader And Providing The Necessary Documentation Regarding The Activities Of The Community Center At His Request. 3) The Contractor Submitting The Offer, In Addition To Working Directly With The Project Participants, Will: A) Perform Its Tasks Diligently, Effectively And On Time, In Accordance With The Established Schedule Of Classes; B) Timely And Properly Implement The Assigned Project Tasks; C) Record Hours And Completed Tasks Regarding The Performed Duties On A Monthly Basis, In Accordance With The Applicable Guidelines On The Eligibility Of Expenses Under The European Funds For Silesia 2021-2027; D) Presented Settlement Documents Immediately After The End Of Each Month; E) Kept Documentation In Accordan
