This Subject Of The Order Is A Framework Agreement Within The Meaning Of Art. 311 Section 1 Of The Public Procurement Law And Specifies The Conditions For The Provision And Implementation By The Contractor Of Waste Collection And Management Services With Code 19 12 10, Hereinafter Referred To As The Subject Of The Order During The Term Of The Contract, Based On Executive Contracts. 2. As Part Of The Subject Of The Order, Waste With Code 19 12 10 Will Be Successively Collected And Managed From The Plant Managed By The Ordering Party, Under Executive Contracts In The Estimated Quantity: 4,000 Mg - Total Amount Of Waste To Be Collected During The Period Of Validity Of The Contract With Z/I Bełchatów, Ul. Przemysłowa14 And 16, Bełchatów. 3. The Waste That Is The Subject Of This Order Will Be Collected From The Ordering Partys Plant And Then Managed In A Manner Consistent With The Applicable Legal Provisions And Annex No. 1 To The Waste Act. 4. Loading Of Waste Is The Responsibility Of The Ordering Party. The Costs Of Transport And Waste Management Are The Responsibility Of The Contractor. 5. The Contractor Will Supervise Loading Activities And Is Obliged To Ensure Appropriate Securing Of The Cargo During Transport. The Contractor Undertakes To Transport Waste To The Destination Without Causing A Threat To Road Traffic Safety, Exceeding The Permissible Weight Of Vehicles Or Exceeding The Permissible Axle Loads. 6. The Scale Installed In Z/I Bełchatów Has A Valid Legalization Certificate. 7. The Waste That Is The Subject Of The Order Constitutes A Waste Stream 19 12 10 With The Following Parameters: A. Fragmentation Fraction - 40 Mm B. Calorific Value - 14,000-16,000 Kj/Kg C. Chlorine Content - 0.8-1.0% D. Moisture - Up To 25% Produced As A Result Of Grinding Large-Sized Waste And Sorting Selectively Collected Waste Packaging, As Well As From The Processing Of Industrial Waste. 8. Waste Collection Will Take Place In The Amount Specified In The Executive Agreement Constituting An Annex To The Swz. The Contractor Is Obliged To Collect And Manage Waste Within The Deadlines Specified Each Time In The Executive Agreement. 9. Waste Collection Will Take Place On The Basis Of The Collection Notification Sent By The Ordering Party To The Contractor. The Ordering Party Allows Applications To Be Submitted Via E-Mail, Fax Or Telephone. After Receiving The Notification, The Contractor Notifies The Waste Collection. The Collection Notification Should Include Information About The Target Installation To Which The Waste Will Be Delivered, The Number Of The Place Of Business (Mpd), Name, Bdo Number And Details Of The Transporting Entity, Date Of Collection, Vehicle Registration Number And The Name And Surname Of The Driver Or Possibly Other Data Necessary To Properly Keep Waste Records, Including Issuing And Confirming Waste Transfer Cards In The Bdo System. 10. The Contractor Undertakes To Submit To The Ordering Party A Copy Of The Decision Granting Permission To Process Waste (Applies To A Decision That Was Changed During The Term Of The Contract) Immediately After Receiving It, No Later Than
