1. The Subject Of The Proceedings Is The Award Of An Order For Successive Deliveries For The Clinical Hospital. Dr. Józefa Babiński Spzoz In Krakow, Pharmaceutical Products (Medicines) In Accordance With The Assortment Specified In The Price Form - In Annex No. 1.1 To Swz.2. In Each Item Of The Price Form, The Contractor Will Provide: Responsible Entity, Gtin Code And Trade Name Of The Product, In Accordance With The Requirements Of The Price Form.3. The Subject Of The Order Will Be Delivered To The Ordering Party In Accordance With The Conditions Specified In The Regulation Of The Minister Of Health Of March 13, 2015 On The Requirements Of Good Distribution Practice.4. The Contractor Is Obliged To Deliver The Subject Of The Order Together With Information Including The Manufacturers Indication, A Description Containing The Manufacturers Indications Regarding The Properties Of The Subject Of The Order, Safety Rules Of Use And Expiration Dates. These Requirements Must Be Included On Or In The Packaging In The Form Of A Leaflet, In Accordance With The Applicable Regulations.5. The Offered Products, Within The Scope Of The Subject Of The Order, Must Have A Valid Registration Certificate, Attestation, Quality Certificate, Declaration Of Conformity Or Authorization For Marketing And Use In Health Care Facilities, As Well As Appropriate Packaging Labeling In Accordance With The Applicable Regulations.6. The Validity Period Of The Products Delivered To The Ordering Party Cannot Be Shorter Than 12 Months From The Date Of Product Delivery.7. Place Of Delivery: Hospital Pharmacy, Located At The Registered Office Of The Ordering Party, I.E. Kraków, Ul. Dr. Józefa Babińskiego 29, Building No. 13. 8. Delivery Of The Subject Of The Order Will Be At The Contractors Own Expense And Risk.9. The Subject Of The Order Will Be Implemented Under The Conditions Specified By The Ordering Party In The Contract Template - Annex No. 4 To The Swz.
