A) Part 1 - Delivery Of An Rr Holter, An Ecg Holter, A Spirometer, The Detailed Description And Scope Of Which Are Determined By:• Assortment And Price Form - Annex No. 4.1 To The Swz Draft Provisions Of The Contract - Annex No. 5 To The Swz 3) The Equipment Must Be New, Unused, And Cannot Be The Subject Of Enforcement Or Administrative Proceedings Or Be The Subject Of Security. 4) Together With The Delivered Assortment, The Contractor Will Provide The Equipment Required For A Given Assortment. Approvals, Certificates, Operating Instructions, Assembly Instructions And Warranty Cards.5) The Contractor Is Obliged To Deliver, Unload, Bring, Unpack And Assemble The Assortment Included In The Subject Of The Contract To The Health Center Building In Żabno At Ul. Al. Piłsudskiego 7, 33-240 Żabno. With Respect To The Assortment Which, Due To Its Nature Or Purpose, Requires Assembly, Setting, Connection Or Commissioning, The Contractor Is Obliged To Perform These Activities And Hand Them Over To The Ordering Party Ready For Use. In Addition, The Contractor Is Obliged To Train The Ordering Partys Staff In The Operation Of The Supplied Equipment And Devices.6) The Ordering Party Expects The Contractor To Provide The Price For The Implementation Of The Subject Of The Order As A Lump Sum Price, Which Includes The Execution Of All Elements Of The Order In Accordance With The Description Included In The Annexes Detailing The Given Part Of The Subject Of The Order For Which The Contractor Is Submitting An Offer.7) The Contractor Is Responsible For Carefully Reading The Order Documents In Order To Estimate, On His Own Responsibility, The Costs And Risks Of Performing The Order.8) All Requirements Specified In The Documents Indicated In Point 2) Above Are Minimum Requirements And Their Fulfillment Is Obligatory. Failure To Meet The Above-Mentioned Minimum Requirements Will Result In Rejection Of The Offer As Inconsistent With The Terms Of The Contract Pursuant To Art. 226 Section 1 Point 5 Of The Public Procurement Law.
