In addition to that amount may be deposited in Bank Account No. 7040213 maintained at Kandy (002) Branch of Bank of Ceylon in favour of "Deputy Director General, National Hospital, Kandy" and clear copy of the Bank Deposit Slip clearly mentioning the name of institution and numbers of the items applying for should be sent by email to [email protected] on the same day and the original of the receipt should be attached to the application and submitted.
