Medical Practice And Related Services – Provision Of Medical Services In The Field Of Additional Medical Care For Employees Of Aplikacje Krytyczne Sp. Z O. O. And Their Families. 1. The Subject Of The Order Is The Provision Of Medical Services In The Field Of Additional Medical Care For Employees Of Aplikacje Krytyczne Sp. Z O. O. And Their Families In The Form Of The Following Packages: 1) Individual Package I - Employee Of The Ordering Party; 2) Partnership Package Ii - An Employee Of The Ordering Party And A Close Person Of The Employee; 3) Family Package Iii - The Ordering Partys Employee And The Employees Close Relatives; The Ordering Party Requires The Contractor To Have A Minimum Number Of 500 Facilities In The Masovian Voivodeship (Own Or Cooperating) And At Least 1,500 Facilities In Other Polish Voivodeships Providing Medical Services. The Number Of Facilities Does Not Include Facilities Offering Services Only In The Field Of Occupational Medicine. The Ordering Party Reserves The Right To Verify The Scope Of Activities Of Individual Facilities. The Contractor Is Obliged To Indicate The Number Of Branches In The List Attached To The Bid Form. Failure To Meet The Above-Mentioned Requirement Will Result In Rejection Of The Offer. The Ordering Party Provides For The Possibility Of Exercising The Option Right. Basic Order: Provision Of Medical Services In The Field Of Additional Medical Care For Employees Of Aplikacje Krytyczne Sp. Z O. O. (Ordering Party) And Their Families For A Period Of 36 Months From The Date Of Signing The Contract. Order Under Option Law: Provision Of Medical Services In The Field Of Additional Medical Care For Employees Of Aplikacje Krytyczne Sp. Z O. O. (Ordering Party) And Their Families For A Maximum Period Of 12 Months From The Date Of Termination Of The Contract. 2. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 To The Swz - Description Of The Subject Of The Order (Opz).
