The Subject Of The Order Is The Provision Of Medical Services In The Field Of Preventive Health Care For Employees Of The City Of Lodz Office, Within A Period Of 48 Months From January 4, 2027, Determined In The Subject Scope: Tests According To The Annex. Valuation Table For Preventive Tests And Vaccinations In 2027-2030: A) Initial - Regarding Newly Employed Employees Before Starting Work For The Employer, After Changing The Job Position And In The Event Of New Exposures, B) Periodic - Relating To Employees Is Mandatory; The Above-Mentioned Tests Will Be Aimed At Determining The Employees Continued Ability To Work In A Given Position And Detecting Any Changes In Health Caused By Work, C) Follow-Up - Regarding Each Employee Who Was On Sick Leave For Longer Than 30 Days; After Performing The Examination, The Preventive Physician Issues An Appropriate Certificate Regarding Health Indications For Work In The Current Position, D) Targeted - They Will Be Performed In A Manner Targeted At A Specific Professional Group Or Disease Entity At The Workplace Due To The Risk. Research Will Include, Among Others: Employees Using A Screen Monitor During Work For At Least Half Of Their Daily Working Time. After Performing The Examination, The Ophthalmologist Will Issue A Certificate In Which - If The Results Of Ophthalmological Examinations Show The Need To Use Glasses While Working On A Screen Monitor - There Will Be A Clear Recommendation That The Employee Should Be Provided With Glasses To Correct Vision.11.6. Contractors Certificate:11.6.1. Obtaining Certification Of Public Procurement Contractors And Using The Certificate Is Voluntary. The Ordering Party Does Not Require A Certificate As A Condition For Applying For The Contract.11.6.2. The Contractor May, Pursuant To Art. 124 Section 2 Of The Public Procurement Law, Instead Of Appropriate Subjective Evidence, Submit A Certificate Confirming The Certification Of Public Procurement Contractors As To Not Being Subject To Exclusion Or Meeting The Conditions For Participation In The Proceedings.11.6.3. The Certificate Replaces Subjective Means Of Evidence Only To The Extent That Its Content Confirms The Grounds For Exclusion Or The Conditions For Participation In The Proceedings Required By The Ordering Party.11.6.4. If The Certificate Does Not Cover All The Grounds For Exclusion Or Participation Conditions Required In These Proceedings, The Contractor Shall Demonstrate The Remaining Circumstances Using Subjective Means Of Evidence Indicated In Swz.11.6.5. The Ordering Party Does Not Require Subjective Means Of Evidence Within The Scope Covered By A Valid Certificate, Subject To Art. 124 Section 4 Pzp, Art. 31 Section 6 Of The Act On The Certification Of Public Procurement Contractors And Circumstances Not Covered By The Statutory Scope Of Certification.11.6.6.
