Tenders Are Invited For Delivery Of Medical Equipment And Supplies As Part Of The Implementation Of The Grant Project Titled Primary Health Care Support

1. Part I – Delivery Of A Portable Autorefractometer.2. A Detailed Description Of Part I Of The Order Can Be Found In Annex No. 2 To The Specifications Of The Terms Of The Order, Hereinafter Referred To As Swz.3. The Descriptions In Annex 2 To The Swz Contain Minimum Requirements For The Device Covered By The Subject Of The Order, Which Means That The Contractor Can Offer A Device With Better Technical Or Operational Parameters.4.In Each Case When The Ordering Party Refers To Standards, European Technical Assessments, Approvals, Certificates, Technical Specifications And Technical Reference Systems In The Swz, Solutions Equivalent To Those Described Are Allowed And The Words Or Equivalent Apply.5.The Delivered Device Must Be Brand New, Manufactured No Earlier Than In 2026, Complete, Approved For Trading And Use, Unused, Not Subject To Any Repairs, Free From Construction, Material, Workmanship And Legal Defects, Fully Functional And Ready For Use In Accordance With Its Intended Purpose, Meeting The Applicable Quality And Technical Standards, Not Encumbered With Any Third-Party Rights And Meeting The Requirements Specified In Swz.6.The Delivered Device, If It Constitutes A Medical Device In Accordance With Applicable Regulations, Must Be Placed On The Market And Used In Accordance With Applicable Law And Have The Ce Marking Required By Law.7.The Contractor Will Perform The Subject Of The Order Using Its Own Tools, Materials, Equipment And Transport. Installation Of The Device Will Involve Performing All Work Necessary To Use It In Accordance With Its Intended Purpose And Function, I.E., Among Others: On Carrying, Setting, Adjusting, Screwing, Starting And Cleaning The Delivery Site. The Costs And Risks Of Transport, Carrying, Packaging, Insurance During Transport And Installation Of The Device Are Borne By The Contractor.8.The Contractor Is Obliged To Provide Training In The Operation Of The Supplied Device For Employees Designated By The Ordering Party. As Part Of The Training, The Contractor Will Provide The Necessary Materials And Persons Competent To Conduct It.9.The Device Will Be Delivered To The Building Of The Municipal Health Center Of The Complex Of Public Health Care Facilities In Olsztynek, To The Room Indicated By The Ordering Party, On The Ordering Partys Working Days, Using The Contractors Own Efforts And At The Contractors Expense.10.The Contractor Is Obliged To Perform The Order In Accordance With The Principles Of Modern Knowledge, Applicable Regulations, Standards And With Respect For Occupational Health And Safety Principles.
