The Subject Of The Task Is The Development Of Daily Forms Of Care Services Provided In The Wolbórz Commune2.The Scope Of The Planned Services Includes: Organization And Provision Of Support For Seniors From The Wolbórz Commune Through The Implementation Of Social Services Provided In The Premises At The Contractors Disposal In The Wolbórz Commune.3.Conducting A Promotional Campaign For The Project, 4.Selection Of Project Participants Among Residents Of The Wolbórz Commune Aged 60+.5.The Activities Carried Out Under The Order Are Intended To Provide Seniors With Access To Activities That Develop Their Independence, Social Activity, Competences, Interests And Integration With The Local Environment. Service Period: From January 2, 2027 To September 30, 2028. Total Service Time: (6 Hours A Day For 20 Days A Month And For 21 Months) - Hours - 2,520.6.The Service Will Include: 1) Providing Second Breakfast And Lunch To Seniors, 2) Transport Of Participants From The Wolbórz Commune, 3) Life Counseling And Individual Support For Seniors - 504 Hours, 4) Manual And Handicraft Classes - 504 Hours, 5) Digital Workshops For Seniors - 504 Hours, 6) Integration And Activation Activities - 504 Hours, 7) Educational Activities For Seniors - 504 Hours.7.The Total Number Of Activities Carried Out As Part Of The Task Is - 2,520 Hours In The Period From January 2, 2027 To September 30, 2028, In Accordance With The Division Of Hours Into Individual Forms Of Support Specified In The Description Of The Subject Of The Order. Detailed Dates, Frequency And Schedule For The Implementation Of Individual Activities Will Be Adapted To The Current Needs Of Participants And Agreed With The Ordering Party. Classes And Support Should Be Implemented In The Wolbórz Commune In A Flexible Manner, Responding To The Actual Needs Of Seniors. And Taking Into Account Their Age, Fitness Level, Psychophysical Capabilities, Level Of Independence, Interests And Social Situation. The Activities Should Cover A Maximum Of 7 Groups Of 12 Seniors From The Wolbórz Commune. The Selection Of Participants Is Made By The Contractor.
