The Subject Of The Order Is The Provision By The Contractor To The Ordering Party Of Medical Services In The Field Of Preventive Health Care For Employees, Residents, Interns And Candidates For Employees And Interns. Part I - Preventive Examinations For Employees Of The Branch In Sosnowiec And Inspectorates In Dąbrowa Górnicza And Jaworzno. The Implementation Of The Subject Of The Order Will Consist In: 1) Performing Preventive Examinations: A) Preliminary, B) Periodic, C) Control, 2) Providing Preventive Health Care Including Medical Examinations Beyond The Dates Resulting From The Frequency Of Periodic Examinations In The Event Of Vision Deterioration; 3) Carrying Out Tests Resulting From The Collective Agreement For Zus Employees, I.E.: - Mammography For Women - Psa For Men; 4) Referring An Occupational Medicine Doctor To Participate In Meetings Of The Occupational Health And Safety Committee And, As Part Of Its Activities, To Periodically Inspect Conditions At Workplaces. 5) Referral Of An Occupational Medicine Doctor To Participate In The Assessment Of Occupational Risk At Workplaces,
