Scope Of Works Covered By The Order: A) Access To Place Of The Order, B) Proper Securing Of Works Carried Out In The Road, C) Taking A Photograph Showing The Animal And The Road Section In A Way That Allows For A Clear Determination Of The Location Of The Order, D) Collecting The Dead Animal Or Its Remains From The Area Of The Road, E) Loading The Dead Animal Or Its Remains Into Appropriate Containers And Onto Means Of Transport, F) Transporting The Dead Animal Or Its Remains To The Treatment Plant For Category 1 Animal By-Products Which Are The Subject Of Order, G) Unloading Of A Dead Animal Or Its Remains, H) Disposal Of A Dead Animal Or Its Remains In A Plant With A Valid Decision Of The District Veterinarian Competent For The Place Of Business Approving Activities In The Field Of Disposal Of Animal By-Products Of Category 1 Material That Are The Subject Of The Order To The Extent Provided For In Art. 12 Of Regulation (Ec) No. 1069/2009 Of The European Parliament And Of The Council Of 21 October 2009 Laying Down Sanitary Rules Regarding Animal By-Products Not Intended For Human Consumption And Repealing Regulation (Ec) No. 1774/2002 (Regulation On Animal By-Products), I) In Each Case Of Reporting Species Subject To The Obligation To Take Biological Samples In Accordance With The Guidelines Of The Veterinary Inspection, The Contractor Is Obliged To Notify The Local District Veterinary Inspectorate,J) In The Event Of Cleaning Up The Carcass Of An Animal Subject To Strict Species Protection In Poland Or An Otter, The Zdw Should Be Immediately Notified By E-Mail,K) Preparation Of The Required Attachments To The Invoice: - Description Of The Location Of The Order Specified By The Number Of The Provincial Road, Mileage Of The Road And The Name Of The Town Within Which The Order Was Performed, - Specification Of The Species Of Animal To Which The Order Was Performed, - Name And Surname Of The Person Person Submitting The Order, - A Copy Of The Commercial Document, - Electronic Photo Documentation Showing The Animal And The Road Section In A Way That Allows For A Clear Determination Of The Location Of The Order. The Above Should Be Delivered To Zdw Before Issuing An Invoice.
