Social Work Services With Accommodation – Housing Routes, Itineraries And Social Support For Integration, 40% Co-Financed By The European Social Fund Plus 2021-2027. The Purpose Of This Contract Is To Determine The Conditions And Terms Under Which The Different Modalities Of The Project “Housing Routes, Itineraries And Social Support For Integration” Will Be Provided For People Who Are Homeless/Street And/Or At Risk Of Social Exclusion In Order To Initiate Processes Of Recovery, Empowerment And Personal Autonomy, Promoting, At The Same Time, Their Social Inclusion With Intensive And Flexible Social Support, Starting From Placement In Housing. This Service Is Inspired By The Housing First, Housing Led And Rapid Rehousing (Early Attention) Methodologies.
