The Subject Of The Order Is To Carry Out Preventive And Diagnostic Tests In Three Key Areas Adapted To The Specific Risks Occurring In The Workplace, According To The Following Specification: Package No. 1: Laboratory Tests, Including In The Field Of Oncological Prevention For Women - Basic Order 45 Pcs., Option Right: Additional 150 Pcs. Package No. 2: Laboratory Tests, Including In The Field Of Oncological Prevention For Men - Basic Order 135 Pcs., Right Of Option: Additional 50 Pcs. Package No. 3: Comprehensive Blood Tests Including Testing To Detect Stress Factors - Basic Order 224 Pcs. (Women And Men In Total), Right Of Option: Additional 140 Pcs. (Women And Men In Total).2.The Place Of Performance Of Services Will Be The Contractors Facilities Indicated In The Offer, Located In At Least Three Of The Mentioned Cities: Katowice, Chorzów, Zabrze, Bytom And Sosnowiec. Facilities Must Be Adapted To The Needs Of People With Disabilities.3 Service Completion Date: The Service Will Be Implemented Successively Throughout The Entire Period From The Date Of Conclusion Of The Contract Until December 31, 2026 Or Until The Maximum Remuneration Amount Is Exhausted, Depending On Which Of These Events Occurs First.
