1) The Subject Of The Order Is The Supply Of Medical Equipment And Supplies For The Needs Of Gpzoz In Łask, Divided Into Parts: A) Delivery Of An Ultrasound Machine (Part 1); B) Delivery Of Small Medical Equipment (Part 2).2) The Price Specified By The Contractor In The Offer Form Should Include All Costs Related To The Execution Of The Subject Of The Order Calculated On The Basis Of Complete Documentation Of The Procedure, Including: A) Contract Template Constituting Annex No. 5 To The Swz, B) Description Of The Subject Of The Order Constituting Annexes No. 2A And 2B To The Swz, Which Is Also A List Of The Required Equipment Parameters.3) The Costs Of The Offer Should Include: - Costs Of Purchasing The Subject Of The Order In Accordance With The Conditions Specified In This Swz And In The Contract Template; - Costs Of Transport To The Place Indicated By The Ordering Party; - Costs Of Insurance For The Time Of Transport To The Place Indicated By The Ordering Party; - Costs Of The Required Software And Equipment; - Costs Of Bringing, Placing, Assembling And/Or Installation; - Costs Resulting From Any Damage Occurring During Transport, Bringing, Arranging And Assembling The Subject Of The Contract, Up To Signing The Acceptance Protocol; - Costs Of The Warranty And Inspections During The Warranty Period; - Costs Of Providing/Providing The Required Documentation And Instructions; - Costs Of Obtaining Documents Confirming The Fulfillment Of Swz Requirements And Parameters Offered By The Contractor In The Offer; - Other Costs Necessary For The Proper Performance Of The Subject Of The Contract.4) The Subject Of The Order Delivered By The Contractor Must Be Brand New (Produced In 2026), Complete, I.E. Ready For Work And Free From Material, Construction And Legal Defects.5) The Subject Of The Order Should Be Admitted To Trading In Accordance With Applicable Regulations.6) The Subject Of The Order Will Be Delivered At The Risk And Expense Of The Contractor By Means Of Transport Guaranteeing Its Proper Quality.7) The Contractor Is Obliged To Provide A Warranty For The Subject Of The Order. 8) Note: The Contractor Is Obliged To Complete And Submit Together With The Offer A Form With A List Of Parameters - Annex No. 2A And 2B To The Swz, Respectively, For Each Part Of The Order, Which Constitutes An Integral Part Of The Offer And Does Not Need To Be Supplemented In The Course Of The Procedure. The List Of Parameters Form Allows The Ordering Party To Verify The Compliance Of The Offer With The Description Of The Subject Of The Contract And Constitutes The Basis For The Evaluation Of Offers. The Ordering Party Provides The Forms In An Editable Version In .Docx Format. The Form Must Be Completed, Signed And Attached To The Offer.
