The Subject Of The Order Is The Provision Of Waste Management Services In The Recovery Process (In Accordance With Annex No. 1 To The Act Of December 14, 2012 On Waste) By The Contractor Of Waste With Code Ex19 12 12, Together With The Service Of Their Successive Collection And Transport From Zakład Zadrożowania Odpadów Olszowa Sp. Z O. O. Located At: Olszowa, Ul. Bursztynowa 55, 63-600 Kępno, To The Place Of Their Development. 2. The Service Should Be Provided In Accordance With The Decisions Held By The Contractor And With All Generally Applicable Legal Provisions, In Particular: A) The Waste Act Of December 14, 2012; B) Act Of April 27, 2001, Protection Law. 3. The Waste Covered By This Order Should Be Transported And Then Managed In Installations Indicated By The Contractor Which: A) Have Valid Decisions To Conduct Waste Recovery Activities Specified In Annex No. 1 To The Act Of December 14, 2012 On Waste In The Scope Of Waste Code 19 12 12; B) They Have The Decisions/Permits/Entries In The Appropriate Registers Required By Law, Authorizing The Transport Of Waste With Code 19 12 12; C) Have An Active Account In The Bdo Electronic System, Enabling Them To Perform The Necessary Activities Related To Waste Records (Confirmation Of Kpo). 4. The Contractor Will Provide The Ordering Party, No Later Than July 5, 2027 And Within 10 Days From The Date Of Receipt Of The Last Batch Of Waste, In Writing And By E-Mail: A) Information Confirming The Type Of Recovery Process Of The Received Waste In Accordance With Annex No. 1 Of The Waste Act Of December 14, 2012; B) The Amount Of Products Or Waste Generated From Them, Indicating The Final Installation To Which They Were Sent (Name, Bdo Number And Mpd Number) Along With The Final Process To Which They Were Subjected. 5. At Each Request Of The Ordering Party, The Contractor Is Obliged To Provide, Within 7 Days From The Request, A Summary Of Data, Containing The Amounts Of Waste Collected And Processed, Along With Information On What Processes They Were Subjected To - In Accordance With Annex No. 3 To The Contract. 6. The Subject Of The Order Is Waste With Code Ex19 12 12 - Ballast After Sorting Waste With Code 20 03 01, Fraction Above 60 Mm. 7. The Waste Will Be Prepared By The Ordering Party For Collection In Loose Form, The Bulk Density Of The Fraction In Question Is Below 200 Kg/M3. 8. The Amount Of Waste With Code Ex19 12 12 During The Term Of The Contract Will Be Approximately 2,500 Mg. This Is An Estimated Quantity, Used To Help Prepare The Offer, And This Quantity May Be Increased Or Decreased. The Actual Amount And Frequency Of Waste Collection Will Depend On The Current Needs Of The Ordering Party. The Ordering Party Informs That The Minimum Amount Of Waste Collected Will Be 100 Mg. The Contractor Is Not Entitled To Any Claims Against The Ordering Party For Failure To Achieve The Estimated Amount Of Waste Specified Above During The Execution Of The Contract.
