The Subject Of The Order Is The Successive Delivery Of Autoimmune And Allergy Testing Kits (Tests) Along With Consumables And The Lease Of Equipment Necessary To Perform Tests For The Provincial Combined Hospital. Dr. Roman Ostrzycki In Konin.2.The Subject Of The Order Was Described In Detail In Annex No. 2 To The Swz (Assortment And Price Form) And In Annex No. 3 To The Swz (Description Of The Subject Of The Order), Which Provide The Quantity Of The Subject Of The Order And A Description Of The Subject Of The Order.3.The Subject Of The Order Will Be Delivered At The Expense, Risk And Transport Of The Contractor. 4. The Conditions For The Implementation Of The Subject Of The Order Are Also Included In The Draft Provisions Of The Agreement, Constituting Annex No. 5 To Swz.5.The Ordering Party Does Not Allow The Submission Of Partial Offers. The Ordering Party Did Not Divide The Order Into Parts Due To The Nature Of The Subject Matter Of The Order; Individual Elements Must Be Compatible With Each Other And Come From One Manufacturer. 6. The Minimum Guaranteed Level Of The Value Of Orders That The Ordering Party Undertakes To Place (Minimum Guaranteed Level Of Use Of The Contract Value) Is Specified In The Draft Provisions Of The Agreement Constituting Annex No. 5 To Swz.7.The Ordering Party Does Not Allow The Submission Of Variant Offers.8.The Ordering Party Allows For The Exercise Of The Option Right. A Detailed Description And Scope Of The Option Right Is Specified In The Draft Provisions Of The Agreement, Constituting Annex No. 5 To The Swz. Provisions Regarding Ict Products, Services And Processes1.If The Subject Matter Of The Contract Includes Or May Include An Ict Product, An Ict Service Or An Ict Process, The Provisions Of This Chapter Shall Apply. 2. The Contractor Is Obliged To Submit, Along With The Offer, A List Of Offered Ict Products, Ict Services And Ict Processes (Ict List), Including Data Enabling Their Clear Identification, In Particular: The Name Of The Solution, Ict Type (Product/Service/Process), Manufacturer Or Supplier, Implementation Model Or Form Of Provision And Function Performed Under The Order.3.The List Of Ict Referred To In Paragraph 2, Should Be Prepared According To The Template Form Constituting Annex No. 6 To The Swz. 4. The Contractor Declares That As Of The Date Of Submission Of The Offer, His Offer Does Not Include: A) The Ict Product, Ict Service Or Ict Process Indicated In The Recommendation Referred To In Art. 33 Section 4 Of The Act Of 5 July 2018 On The National Cybersecurity System, Stating Their Negative Impact On The Fundamental Interest Of State Security, B) An Ict Product, The Type Of Which Was Specified In The Decision On Recognizing The Supplier As A High-Risk Supplier, Referred To In Art. 67B Section 15 Of The Act Of 5 July 2018 On The National Cybersecurity System, Nor The Ict Service Or Ict Process Specified In This Decision.5.It Is Unacceptable To Offer Ict Products, Ict Services Or Ict Proces
