The Subject Of The Order Is The Provision Of Services In The Field Of Collection, Transport And Disposal Of Medical Waste Classified As Hazardous In Group 180102, 180103, 180104, 180108, 180109. 2. The Contractor Is Obliged To Collect Waste From Places Of Storage, I.E.: A) From The Waste Warehouse Located In The Building Of The Rss Hospital Latawiec In Świdnica Ul. Leśna 27-29, Collection Three Times A Week, I.E. On Monday, Wednesday, Friday From From 7:30 A.M. To 1:00 P.M. B) From The Waste Warehouse Located In The Hospital In Żarów, Ul. Wojska Polskiego 7, Collection Once A Week On Wednesday Until 13:00.3. If The Scheduled Waste Collection Date Falls On A Holiday Or Public Holiday, The Contractor Should Provide The Nearest Possible Waste Collection Date In Advance, No Longer Than 72 Hours. Interval Between Receptions.4. The Ordering Party Provides For The Possibility Of Additional Waste Collection Outside The Schedule, After Prior Arrangement Of The Date. 5. Estimated Monthly Amount Of Waste Approx. 13,000 Kg.6. Contractors Obligations: A) Loading And Unloading Waste, B) Weighing Waste In The Presence Of The Ordering Partys Representative, C) Each Disinfection Of Waste Storage Rooms And Sulo Waste Containers, Confirmed By Stickers With The Date Of Disinfection And The Name Of The Agent Used, D) Each Time Confirmation Of Waste Collection On Forms In Accordance With Applicable Regulations - Waste Transfer Card.7.
