The Subject Of The Order Is The Purchase Of Sports And Recreational Services For People Employed At The Masovian Provincial Hospital. Saint Jana Pawła Ii In Siedlce Spółka Z O. O., Their Children And Other Persons Authorized On The Basis Of Personal Access Cards Issued By The Contractor In Sports And Recreational Facilities In Poland, Obligatorily In The Cities Of: Siedlce, Mińsk Mazowiecki, Sokołów Podlaski, Łuków, Garwolin. The Services That Are The Subject Of The Order Will Be Provided In Two Variants: 1) Unlimited Number Of Entries In A Given Calendar Month (Service Open Nature), The Possibility Of Using The Same Facility Several Times A Day And An Unlimited Number Of Different Facilities In One Day; 2) Limited Number Of Entries In A Given Calendar Month (Minimum 8 Entries In A Given Calendar Month). Possibility Of Using The Limited Card By An Authorized Person Several Times On The Same Day Without Declaring The Use Of A Specific Location. The Contractor Will Provide Access To Sports And Recreation Facilities By Issuing And Delivering Personalized Access Cards To The Ordering Party Without Additional Fees.
