1) The Subject Of The Order Is The Provision Of Services For Conducting Specialized Classes: Hand Therapy For 30 Children For Whom The Need For Specific Types Of Classes Was Diagnosed Among The Participants Of The Project Entitled: Our Base - A Space For The Development Of Children And Youth. The Size Of Individual Groups Will Depend On The Diagnosed Needs Of Children.2) Detailed Description Of The Subject Of The Order: The Subject Of The Order Is Hand Therapy Carried Out Individually Or In Groups, Carried Out 1 Hour A Week On A Designated Day Of The Week, At A Designated Time, Between 1:00 P.M. And 5:00 P.M. - Repeated Cyclically Every Week, From The Date Of Signing The Contract Until December 18, 2026. Total Number Of Hours 14.3) Requirements For The Contractor: Before Signing The Public Procurement Contract, The Contractor Shall Provide The Ordering Party With Documents Of The Persons Conducting The Workshops/Classes/Training/Therapy/Psychological Support Confirming That These Persons Have Not Been Convicted By A Final Court Judgment (Certificate From The National Criminal Register And The Register Of Sexual Offenders). The Contractor Shall Provide The Contractor With The Documents Of The Persons Conducting The Workshops/Classes/Training/Therapy/Psychological Support Before Signing The Public Procurement Contract. Workshops/Classes/Training/Therapy/Psychological Support Confirming That The Persons Meet The Requirements Specified In The Conditions Of Participation In The Proceedings Specified In Chapter Ii, Point 7.4) B) Swz. The Contractor Is Obliged To Keep A Log Of Classes As Documentation Confirming The Participants Presence At The Classes And Covering The Subject Of The Workshops/Classes/Training/Therapy/Psychological Support And The Number Of Hours Completed. On The Documentation Related To The Implementation Of Workshops/Classes/Training/Therapy/Psychological Support, The Contractor Will Use Logos Provided By The Ordering Party Informing About The Financing Of The Classes From European Union Funds. The Contractor Undertakes To Carry Out The Task In Accordance With: • The Principles Of Equal Opportunities For Women And Men, • The Principle Of Accessibility For People With Disabilities, • The Principle Of Non-Discrimination And Respect For Diversity. The Ordering Party Requires That The Workshops/Classes/Training/Therapy/Psychological Support Carried Out Under This Order Are Accessible To People With Disabilities And Designed In Accordance With The Principles Of Universal Design Referred To In Art. 100 Paragraph 1 Of The Public Procurement Law. The Contractor Is Obliged To Provide Appropriate Organizational, Technical And Communication Solutions Enabling People With Various Types Of Disabilities To Participate In Classes, In Particular By: - Adapting Teaching Materials To The Needs Of People With Disabilities (E.G. Materials In Digitally Accessible Formats, The Possibility Of Enlarging The Text, Contrast), - Ensuring Communication Accessib
