Diagnostic Agents – Vz0257224: Procurement Of 30 Glucometers With Docking Stations, Including Provision Of Service And Supplies Of Consumables - Repeat. • The Subject Of The Performance Of The Public Order (Hereinafter Also Referred To As Vz) Is The Supply Of 30 Glucometers With Docking Stations, Including The Provision Of Complete Service For The Duration Of The Contract, But At Least For A Period Of 5 Years, And The Supply Of Consumables Incl. Accessories Necessary To Operate The Supplied Devices, All Min. For A Period Of 60 Months With The Possibility Of An Indefinite Extension. The Public Order Will Include The Installation, Assembly And Commissioning Of The Delivered Equipment, The Execution Of Briefings In Accordance With The Relevant Provisions Of Act No. 375/2022 Coll. On Medical Devices (Hereinafter Referred To As Act No. 375/2022). The Delivery, Installation And Commissioning Of The Equipment At The Customers Headquarters Must Be Carried Out In Accordance With The Applicable Legal Regulations Of The Czech Republic And Standards And In Accordance With Generally Binding Regulations And The Manufacturers Recommended Methodologies. Part Of The Scope Of The Contract Will Be The Performance Of Pbtk (Periodic Safety And Technical Inspections) Of The Supplied Devices For The Duration Of The Service Contract (According To Act No. 375/2022 Coll.) As Well As Revisions (Whether Prescribed By The Manufacturer, The Domestic Service Organization Or Legal Regulations) And Technical (User) Support And Service, Including The Supply Of All Spare Parts For The Duration Of The Service Contract. If Consumables Are Required To Perform Any Other Prescribed Test Performed By The Supplier, They Are Always Included In The Performance Of This Inspection And Therefore Cannot Be Separately Billed. The Subject Of The Performance Of The Public Contract Is Specified In The Detailed Technical Specification, Which Forms Annex No. 2 Of The Procurement Documentation. • Part Of The Required Subject Of The Performance Of The Public Contract Is Also The Continuous Supply Of Consumable Medical Supplies Listed In Annex No. 1 Of The Procurement Documentation Required For The Full Operation Of The Supplied Equipment, According To The Current Needs Of The Contracting Authority (The Assumed Scope Of The Contracting Authoritys Needs Is Given In Appendix No. 1 Of The Procurement Documentation), All For The Duration Of The Service Contract. • The Subject Of The Performance Of The Public Contract Is Also Defined By The Binding Draft Of The Purchase Contract, The Contract For Recurring Performance And The Service Contract, Which Form Annexes No. 4, 5 And 6 Of The Tender Conditions. • The Quantity Of Anticipated Analyses, Which Is Stated In Annex No. 1 Of The Procurement Documentation – Price Calculation Diagnostics, Is An Indicative Quantity, Or Assumed. The Contracting Authority Is Entitled To Determine The Specific Quantity And Time Of Fulfillment Of Individual Partial Supplies Of Consumables According To Its Immediate Or Current Needs With Regard To The Composition Of Patients Without Penalty Or Other Penalty On The Part Of The Participant (Supplier).
