The Subject Of The Order Is The Provision Of Cemetery Services Consisting Of: 1. Administration Of The Municipal Cemetery In Rybnik - Boguszowice Stare Located At Ul. Zadumy 33 With An Area Of 37,246 M² Together With A Plot Of Land With An Area Of 642 M² And A Funeral Home With A Usable Area Of 134 M² In The Following Scope: 1) Providing Access To Areas For Graves And Collecting Cemetery Fees In The Amounts In Accordance With The Applicable Price List Approved By The Mayor Of Rybnik.2) Recording Cash Collected From Cemetery Fees At The Cash Register Fiscal Of The Ordering Party And Making Payments To The Ordering Partys Account.3) Payment By The Contractor To The Ordering Party Of Fees Related To The Operation Of The Property, I.E. For Electricity, For Supplied Water In The Amount Specified In The Issued Vat Invoice And Payment Of Rent In The Net Amount Of Pln 5,000.00 For The Building Of The Funeral Home And The Area Of The Municipal Cemetery In Accordance With The Issued Vat Invoice;4) The Contractors Own Payment Of Fees Related To :A) Cleaning Of Septic Tanks,B) Disposal Of Medical Wastec) Waste Arising From Business Activities And Green Waste,D) Inspection Of Buildings (Funeral House And Bell Tower) Under The Supervision Of The Ordering Partys Representative.5) Compliance With The Arrangement Of The Municipal Cemetery In Accordance With The Cemetery Development Plan.6) Supervision Of The Use Of The Municipal Cemetery, In Particular The Burial And Exhumation Of The Deceased, Placing Tombstones And Construction Of Tombs In Accordance With The Law;7) Correct Management Of Burial Places And Proper Verification Of The Person Reporting Burial In An Existing Grave, In Particular: A) Including Cemetery Books, Data Regarding The Grave And Persons Who Have Previously Reported Or Performed Burials In A Given Grave. B) Determining The Identity Of The Person Reporting The Burial And Verifying His Or Her Authorization To Perform Burial In A Given Grave, Taking Into Account The Provisions Of The Act Of January 31, 1959 On Cemeteries And Burial Of The Dead, In Particular Article 10.C) Request, In The Absence Of Clear Data In The Cemetery Documentation, Documents Or Written Declarations Enabling The Establishment Of Kinship, Affinity Or Other Title Authorizing The Burial. Burial In A Given Grave - Suspension Of Activities Related To The Burial Until Doubts Are Clarified And Documents Confirming Rights Are Obtained.8) Maintaining Cemetery Documentation (Book Of People Buried In The Cemetery, Grave Book, Alphabetical List Of People Buried In The Cemetery In A Yearly Order);9) Keeping A Register Of Fees And Reservations Extending The Grave Fee For The Next 20 Years;10) Monthly Submission Of Death Cards, Death Certificates, Cremation Certificates And Permits For The Import Of Deceased Persons From Abroad; 11) Annual Submission Of Sketches Of Tombstones, Ongoing Updating Of Photos Of Completed Tombstones And New Burials - Entering Them Into The Interactive Cemeter
