The Implementation Will Include Installation Of The Equipment In The Place Indicated By The Ordering Party, Initialization Of The Equipment, Including Assigning Ip Addresses To The Management Interfaces, Connecting The Delivered Servers To The Matrix And The Ordering Partys Lan, Configuration Of The Matrix Resources Consisting In Providing Disk Resources To The Delivered Servers And Preparation Of Post-Implementation Documentation Containing: A List Of Assigned Addresses, Established Users, Passwords And Accounts.2. The Subject Of The Order Is Implemented On The Basis Of A Grant Agreement, Financed From The Budget Of The Ministry Of Health As Part Of The Implementation Of The Task Entitled: Purchase Of It Infrastructure For The Needs Of The Mental Health Center Of The Provincial Independent Complex Of Public Health Care Facilities. The Subject Of The Order Referred To In Section 1 Must Be: 1) Brand New, 2) Manufactured No Earlier Than 6 Months Before The Date Of Delivery To The Ordering Party And Come From The Manufacturers Authorized Distribution Channel - Applies To Servers, While The Matrix Must Be Produced No Earlier Than 9 Months Before The Date Of Delivery To The Ordering Party And Come From The Manufacturers Authorized Distribution Channel, 3) Covered By The Manufacturers Service In The Republic Of Poland, 4) Be On The Windows Server Catalog List And Have The Certified For Windows Status For Windows Server Systems 2022, Windows Server 2025, 5) Originally Packed, Showing No Signs Of Use, 6) Undamaged, 7) Professionally Made, Maintaining High Quality, Aesthetics And Durability, 8) Delivered Complete And In Its Entirety, Ready For Operation After Implementation, As Intended, Without Any Additional Purchases, 9) Consistent With The Technical Parameters Specified In The Description Of The Subject Of The Order Constituting Annex No. 1A To Swz.4. The Contractor Is Obliged To Complete The Subject Of The Order On The Terms And Conditions Described In The Swz And In The Draft Provisions Of The Contract Constituting Annex No. 2 To The Swz, The Contractors Offer Form Constituting Annex No. 1 To This Swz/Annex No. 1 To The Contract (An Integral Part Of The Contract), The Description Of The Subject Of The Contract - Technical Parameters Of Computer Devices - Constituting Annex 1A To This Swz And Other Documents Affecting The Proper Implementation Of The Subject Of This Contract.5. The Assortment Covered By The Order Must Have The Legally Required Certificates, Approvals, Certificates Of Approval For Use/Declarations Of Conformity Or Other Documentation Confirming That The Offered Equipment Is Approved For Use On The Polish Market.
