The Subject Of The Order Is The Provision Of A Transport Service Consisting In Regular Transport Of Elderly People Who Are Participants Of The Project Entitled: Social Activity And Integration Of Residents Of The Ulhówek Commune No. Felu.08.09-Iz.00-0048/24, Implemented Under The European Funds For Lubelskie 2021-2027 Program, Co-Financed By The European Social Fund Plus. The Project Participants Are 50 Natural Persons Living, Within The Meaning Of The Civil Code, In The Ulhówek Commune, In The Following Towns: • Żerniki, • Hubinek, • Rzeczyca, • Tarnoszyn, • Szczepiatyn, • Dębina.These Are Elderly People Who, Due To Age, Health Condition Or Disability, Require Support In The Field Of Transport. A) The Service Includes Transport Of Project Participants From Their Place Of Residence To Animation Classes Held In Ulhówek And Transport To Their Place Of Residence After The Classes. B) Transport Will Be Carried Out On Average 4 Times A Week, About 16 Times A Month, On Working Days, In Accordance With The Schedule Provided By The Ordering Party. The Ordering Party Allows For The Possibility Of Changing The Hours And Days Of Transport Resulting From The Organization Of Classes Or Other Project Events. C) The Estimated Mileage Is Approximately 90 Km Per Day. The Total Number Of Kilometers Is Indicative And May Change Depending On The Number Of Participants Using The Transport, Their Place Of Residence And The Current Needs Of The Project, While The Reduction In The Total Number Of Kilometers To Be Completed Over The Entire Duration Of The Contract Will Be No Greater Than 20% In Relation To The Amount Indicated In This Description Of The Subject Of The Order. D) The Contractor Is Obliged To Ensure Full Readiness To Provide The Service In The Entire Ulhówek Commune, Including The Towns Of: Żerniki, Hubinek, Rzeczyca, Tarnoszyn, Szczepiatyn And Dębina.E) The Contractor Will Provide One Vehicle Adapted To The Transport Of People With Disabilities, Meeting The Applicable Technical And Safety Requirements And Enabling The Safe Transport Of People With Reduced Mobility, And This Vehicle Must Provide Such A Number Of Seats That Will Enable The Transport Of 50 People At The Same Time.F) The Driver Will Be Obliged To Assist Participants When Getting In And Out Of The Vehicle And Ensure Safe Transport Throughout The Duration Of The Service.G) The Contractor Will Drive There Will Be A Time Card Containing The Date Of The Journey, The Number Of Kilometers Traveled And Confirmation Of The Service Provided By The Ordering Partys Representative. The Time Card Will Constitute The Basis For The Monthly Settlement. H) The Travel Routes Will Be Determined By The Contractor In A Way That Ensures The Shortest Possible Travel Time And Optimization Of The Number Of Kilometers Traveled, Taking Into Account The Participants Place Of Residence.
