The Subject Of The Order Is The Supply Of Medicines For The Needs Of Patients In The Departments Of The Lung Diseases Hospital In Orzesze For 2026. The Order Includes Medicines Used Under The B6 Drug Program, Specified In The Assortment And Price Form, Constituting Annex No. 2 To The Swz. The Detailed Scope Of The Subject Of The Order, Including The Type Of Medicinal Products, Active Substances, Doses, Pharmaceutical Form, Suggested Packaging And Expected Quantities, Is Specified In The Assortment And Price Form - Annex No. 2 To The Swz. 4. Terms Of Delivery 1) The Contractor Undertakes To Deliver The Subject Of The Order, I.E. Medicines Used Under The B6 Drug Program, Specified In The Assortment And Price Form - Annex No. 2 To The Swz: A) Within 24 Hours Of Placing The Order By The Ordering Party; B) In The Case Of Ordering A Medicine Urgently, The So-Called Cito - Within A Maximum Of 12 Hours From The Date Of Placing The Order By The Ordering Party; C) On Working Days, From 7:00 A.M. To 2:00 P.M. 2) Orders Will Be Placed By The Ordering Party Via E-Mail Or By Phone. 3) Deliveries Will Be Carried Out By The Contractors Transport, At His Expense And Risk, In Accordance With The Principles Set Out In The Draft Provisions Of The Agreement, Constituting Annex No. 6 To The Swz. 5. Equivalent Medicinal Products 1) The Ordering Party Allows The Offer Of Medicinal Products Equivalent To The Products Specified In The Assortment And Price Form - Annex No. 2 To The Swz, Provided That The Offered Product Meets All The Requirements Specified By The Ordering Party. 2) The Ordering Party Will Consider An Equivalent Medicinal Product To Be A Product That Contains: A) The Same Active Substance; B) The Same Dose Of Active Substance; (C) The Same Pharmaceutical Form; D) The Same Route Of Administration; E) Properties Allowing Its Use In Accordance With The Intended Purpose Specified By The Ordering Party, In Particular As Part Of The **B6 Drug Program; F) Marketing Authorization Required By Law As A Medicinal Product In The Territory Of The Republic Of Poland. 3) The Ordering Party Allows The Medicinal Product To Be Offered In A Commercial Packaging Other Than That Indicated In The Assortment And Price Form, Provided That The Required Dose Of The Active Substance Is Maintained And The Quantity Of The Product Corresponding To The Ordering Partys Needs Is Ensured. 4) If A Medicinal Product Is Offered In A Different Commercial Packaging, The Contractor Is Obliged To Appropriately Recalculate The Number Of Packages Offered So As To Provide The Ordering Party With A Quantity Of The Medicinal Product Corresponding To The Demand Specified In The Assortment And Price Form. 5) The Ordering Party Allows A Different Size Of Commercial Packaging Than That Indicated In The Assortment And Price Form, With An Appropriate Conversion Of The Number Of Packages Ordered. 6) The Contractor Offering An Equivalent Product Is Obliged To Indicate In The Assortment And Price Form The Trade Name Of The Offered Product, The Manufacturer And The Ean Code, If The Product Has Such A Code. 7)
