The Subject Of The Order Part 1 - Delivery Of Medicines And Medical Supplies Is The Supply Of Medicines And Medical Supplies For The Needs Of Residents Of Dps And Its Branches, Delivered On The Basis Of A Prescription Issued By A Health Insurance Doctor Or A Request Submitted By The Ordering Party. 1) The Ordering Party Does Not Allow Offering Substitutes For The Drugs Indicated In The Price Form. 2) Places Of Delivery: Dps Headquarters In Krasnystaw At Ul. Kwiatowa 1 And Dps Branches In Stężyca Nadwieprzańska, Ostrów Krupski And Bzowiec.3) Frequency Of Deliveries: Due To The Service Of A Total Of Four Facilities That Are Part Of Dps Krasnystaw, Deliveries Take Place: From Monday To Friday Inclusive, From 7:00 A.M. To 7:00 P.M.; On Saturdays From 7:00 A.M. To 1:00 P.M. 4) The Ordering Party Requires The Contractor To Indicate The Address Of The Pharmacy That Will Fill The Ordering Partys Prescriptions. 5) The Contractor Fulfilling The Order Is Obliged To Issue Personal Invoices For The Purchased Medicines (In The Name Of The Patient - A Resident Of The Dps). The Contractor Will Prepare A Specification For Each Invoice, Including The Payment Amounts, Up To And Above The Price Limit, In Accordance With The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2023, Item 901, As Amended).6) The Contractor Executing The Order Is Obliged To Pack Medications For Each Patient Separately, In Accordance With The Doctors Prescription. Each Delivery Of Medicines Must Be Confirmed With An Attached Invoice On The Day Of Delivery. The Ordering Party Requires That Each Batch Of Invoices Be Accompanied By A List (List) Of Invoices Relating To A Given Delivery. 7) The Ordering Party May Exercise The Option Right While Maintaining The Following Rules: A) The Ordering Party May Increase The Quantity Of Selected Selected Products Listed In The Price Form, Which Is Annex No. 2 To The Swz, While Maintaining Their Unit Prices. B) The Maximum Value Of The Option Is 10% Of The Net Order Value.
