1. The Subject Of The Order Is A Service Consisting In Carrying Out Individual Physiotherapy For Residents Of The Paradyż Commune Who Have A Referral From The Referring Doctor.2.The Service Will Include Individual Therapy For Each Resident Of The Paradyż Commune.3.The Tasks Will Be Carried Out From Monday To Friday, 7.5 Hours A Day.4.Physiotherapy Treatments Are To Be Performed By 2 People Who Have: A.Appropriate Qualifications (Qualifications) Of A Physiotherapist In Accordance With The Act Of September 25, 2015. About The Profession Of A Physiotherapist; B.Entry Into The National Register Of Physiotherapists; C.At Least 1 Person Has One Year Of Experience As A Physiotherapist;5.Employment Conditions: Self-Employment6.When Providing Physiotherapy Treatments To Patients, The Contractor Will Use Free Of Charge: A) The Premises At Street Opoczno 4 Made Available By The Ordering Party; B) Medical Apparatus And Equipment Of The Ordering Party; C) Pharmaceuticals And Disposable Equipment Of The Ordering Party;7.The Means Specified In Point 6 May Be Used Only To The Extent Necessary To Provide The Treatments That Are The Subject Of The Order.8.The Contractor May Not Use The Funds Specified In Point 6 For The Purposes Of Providing Paid Physiotherapy Treatments.9.Paid Services Are Not Allowed.10.The Contractor Will Perform Treatments In The Field Of: - Electrotherapy; - Phototherapy; - Magnetotherapy; - Ultrasounds; - Laser Biostimulation; - Kinesiotherapy; - Use Of Equipment: Ugul; - Massage.11.The Contractor Must Have A Computer To Keep Medical Records And Maintain A Patient Card And Confirm The Performance Of Physiotherapy Treatments In Written And Electronic Form12.Maintaining Order And Cleanliness At The Workplace;13.Taking Care Of The Equipment And The Treatment Station (Washing, Disinfecting);14.Treating The Patient With Due Respect And In A Polite Manner;15.Not Exceeding Ones Professional Skills In Performing Treatments Where The Scope Of The Treatments Exceeds The Ability To Refer To More Competent Physiotherapists;16.Guided By The Patients Good, Respecting His Or Her Subjectivity And Personal Dignity.
