The Subject Of The Order Is The Delivery Of A Digital Screening Mammogram To The Lower Silesian Oncology Center For Pulmonology And Hematology In Wrocław, Together With The Necessary Additional Equipment, Installation, Commissioning, Training Of The Ordering Partys Staff And Installation Of Air Conditioning In The Laboratory Room. 2. The Order Is Co-Financed From The Funds Of The Wrocław Commune As Part Of The Public Task Entitled Investment Purchases Based On Subsidy Agreement No. 2/D/Zi/2026 Of June 22, 2026. 3. The Subject Of The Order Should Be Delivered To Dwomp Ul. Joannitów 10 – 12, 50-525 Wrocław, To Room No. 210 Located On The Second Floor Of The Building, Hereinafter Referred To As The Studio. The Room Plan Is Attached As Appendix No. 13 To The Swz. 4. The Subject Of The Order Includes: 1) Preparation And Delivery Of A Radiological Protection Design For The X-Ray Laboratory In Question, Together With A Design And Description Of Shields Against Ionizing Radiation And Ventilation, Indicating The Fulfillment Of The Conditions For Nuclear Safety And Radiological Protection, Taking Into Account The Mammography Device Provided For Use In This Laboratory, 2) If The Developed Or Design Results In The Need To Construct Additional Radiological Shields, The Contractor Will Be Obliged To Make These Shields As Part Of The Contractual Expediency, 3) Execution Wiring Powering The Offered Mammography Device From The Nearest Switchboard, 4) Installing A Warning Light Signal Placed Above Each Door To The Office, Switched On Simultaneously With The Generator Power Supply. Inscriptions On The Luminaires In Accordance With The Radiological Protection Design Or Ior Guidelines, 5) Execution Of A Cooling Installation, Including Installation Of An Air Conditioner (Indoor And Outdoor Units) In The Room With A Power Adjusted To The Needs Of The Supplied Device Based On Information Obtained After An On-Site Visit, 6) Integration Of The Devices Delivered As Part Of The Order With The Ordering Partys Existing It Systems. 7) Training Of Employees In The Work And Operation Of The Mammography Machine And The Technical Installations Operating Within The Laboratory, Including Air Conditioning. 8) The Contractors Participation In The Final Acceptance Of The Laboratory By Pwis, Related To Obtaining Consent To Conduct Medical Activities In The Mammography Laboratory In Question. 5. Launching The Subject Of The Order, Which The Ordering Party Understands As The Possibility Of Providing Health Services In The Mammography Laboratory In Question. 6. The Ordering Party Will Perform The Construction Finishing Works Of The Studio On Its Own In Accordance With The Contractors Guidelines. 7. A Detailed Description Of The Requirements Is Included In Annex No. 1 To The Swz And In Annex To Swz No. 8 Draft Contract. 8. The Parameters Described In Annex 1 To The Swz As Required (Parameters Without Scoring) Constitute Boundary Requirements, Failure To Meet Which Will Result In Rejection Of The Offer. 6.5.
