1. The Subject Of The Order Is The Modernization And Adaptation To Sanitary And Hygienic Requirements Of The Medical Waste Storage Room At The Headquarters Of Mazowiecki Centrum Stomatologii Sp. Z O. O. In Warsaw At Ul. Nowy Zjazd 1 (Case No. Zp/20/26).2.As Part Of The Order, The Contractor Will Be Obliged To Develop Design Documentation Required For The Implementation Of The Subject Of The Order And To Obtain Legally Required Permits, Notifications, Arrangements And Opinions, If Required, And Then To Perform Construction Works In Accordance With Applicable Regulations, In Particular The Construction Law Act. Detailed Requirements In This Respect Are Specified In The Pfu (Hereinafter Referred To As The Pfu) Constituting Annex No. 1 To Swz.3.The Detailed Scope Of The Subject Of The Order And The Quality Requirements Are Specified In The Functional And Utility Program Constituting Annex No. 1 To Swz.4.The Ordering Party, Pursuant To Art. 131 Section 2 Point 1 Of The Act, Requires The Submission Of An Offer After The Contractor Has Visited The Site Of The Contract Execution. Due To The Specificity Of The Subject Of The Order, The On-Site Visit Is To Enable The Contractors To Become Acquainted With The Existing Condition Of The Room, Technical And Organizational Conditions And The Conditions For Carrying Out The Works. The Site Visit Will Take Place On September 10, 2026 At 11:00 - 3Rd Floor, Room 318. Participation In The Site Visit Is Obligatory. An Offer Submitted Without The Required On-Site Visit Is Subject To Rejection Pursuant To Art. 226 Section 1 Point 18 Of The Act.5. The Ordering Party Requires The Offer To Be Secured With A Deposit. The Contractor Is Obliged To Pay A Deposit Of Pln 2,000.00 (In Words: Two Thousand Zlotys 00/100).
