The Subject Of The Order Is: Provision Of Care Services In The Place Indicated By The Ordering Party For The Charges Of The Social Services Center In Głogów, In The Period From October 1, 2026. Until September 30, 2027. Detailed And Quantitative Scope Of The Subject Of The Order: It Is Planned To Provide Care Services In A Total Number Of Approximately 55,200 Full Clock Hours In The Period From October 1, 2026 To September 30, 2027, In The City Of Głogów. The Above Service Will Be Provided Under The Following Conditions: A) The Order Covers The Provision Of Approximately 4 Care Services 600 Full Clock Hours Per Month, Which Gives A Total Number Of Approximately 55,200 Full Clock Hours In The Period From October 1, 2026 To September 30, 2027 (12 Months) In The Place Indicated By The Ordering Party, In The City Of Głogów, To A Person Who Requires Help From Other People Due To Age, Illness Or Other Reasons, If He Or She Is Alone Or Deprived Of The Assistance He/She Requires Or If His/Her Family Also Requires Assistance The Non-Resident Spouse, Ascendants And Descendants Cannot Provide The Assistance Required. B) The Number Of Hours Of Care Services Specified In The Swz Is An Estimated Amount And May Be Reduced Or Increased, Provided That The Total Value Of Changes In The Gross Price Of The Subject Of The Order Does Not Exceed 15%. Changes In The Number Of Hours As Well As A Reduction Or Increase In The Value Of The Contract Do Not Require An Annex To The Contract Preparatory Work, E.G. Travel To The Client, C) The Contractor Declares That He Will Not Pursue Financial Claims Regarding A Reduced Number Of Care Services Provided, D) The Number Of Hours Of Care Services Depends On The Needs Of The Residents. Persons Performing Care Services Must Meet The Requirements Referred To In Article 50(4A) Of The Act Of 12 March 2004 On Social Assistance, I.E. They Must Be A Person Who: - Is An Adult, - Is Not A Member. Family Of The Person For Whom Care Services Are Provided, - Is Not A Spouse, Ascendant Or Descendant Of The Person To Whom Care Services Are Provided, Living Separately, - Has Submitted To The Entity Providing Care Services A Declaration That She Is Mentally And Physically Capable Of Providing Care Services, - Has Completed Training In The Field Of First Aid. Care, - Respect For The Privacy And Personal Dignity Of The Person For Whom Care Services Are Provided, And Respecting His Or Her Right To Autonomy In Decision-Making And Self-Determination, - Care For The Safety Of The Care Services Provided By Observing Hygiene Rules. - Confirmation Of The Activities Performed By The Person, Through The Clients Signature On The Activity Order Card (Appendix No. B To The Draft Contract) For Whom Care Services Are Provided. Detailed Procedure For The Implementation Of Care Services And Rights And Obligations The Ordering Party And The Contractor During The Provision Of Care Services Were Included In The Draft Contract Constituting Annex No. 8 To This Swz.
