Tenders Are Invited For Providing Senior Assistant Services To Residents Of The Social Welfare Center In Ożarów In 2027-2029

1. The Subject Of The Order Is To Provide The Services Of An Assistant To An Elderly Person (60 Years And Over) - A Ward Of The Social Welfare Center In Ożarów, Hereinafter Referred To As The Assistant, Consisting In Assistance In Performing Basic Everyday Activities Such As Preparing Meals, Shopping, Keeping Order In The Apartment, Support In Self-Service Activities (E.G. Dressing, Personal Hygiene), Moving Around, Dealing With Official And Life Matters, Shopping, Using Services And Infrastructure Public, As Well As In Reaching Educational Institutions, Rehabilitation, Cultural Institutions And Other Places Of Social Activity. The Assistant Will Also Support Participants In Taking Up Activities And Social Contacts, Increasing Their Independence, Sense Of Security And Ability To Make Decisions About Their Own Lives. The Assistants Services Will Be Intended For Older People With Special Needs, Hereinafter Referred To As Participants, Who Need Support In Everyday Functioning, Leaving Home, Dealing With Official Matters Or Maintaining Social Activity, Without The Need To Provide Constant Care. The Services Will Be Entirely Provided By Pes (Social Economy Entity). If A Partial Service Is Ordered, The Subcontractor Must Meet All The Requirements As A Contractor, Including Being A Social Economy Entity, In Accordance With The Above. Act.2. Description Of The Subject Of The Order:1) The Elderly Persons Assistant Service Will Be Provided In The Ożarów Commune.2) The Elderly Persons Assistant Service Will Be Provided In The Period From January 4, 2027 To April 30, 2029 To Persons Who Have Been Granted This Service.3) The Above-Mentioned Assistance Services Will Be Provided In The Elderly Persons Place Of Residence In The Ożarów Commune.4) The Contracting Authority Understands An Hour Of Service Provision As An Hour. Clock Provided By The Patient.5) The Assistance Service May Be Provided 12 Hours A Day, 7 Days A Week From 7:00 A.M. To 7:00 P.M., And By The Same Assistant For A Maximum Of 12 Hours A Day. A 24-Hour Period Should Be Understood As 24 Consecutive Hours, Starting From The Hour At Which The Assistant Starts Providing The Assistance Service. The Time Needed To Provide The Assistance Service Does Not Include Travel To And From The Participant.3. The Expected Total Number Of Hours Of Senior Assistance Services To Be Performed Is 13,920 Hours. Assistance For The Elderly Will Be Provided To At Least 30 People. The Average Number Of Hours Per Participant Is 464 Hours.4. The Ordering Party Reserves The Following: 1) The Possibility Of Providing A Smaller Number Of Hours For Reasons Beyond Its Control (E.G. Reducing The Number Of People Using The Assistance Service, Failure To Use The Allocated Limit Due To Illness, Resignation, Death Of The Participant). 2) Ordering The Service For A Smaller Number Of Hours Cannot Be The Basis For Any Claims On The Part Of The Contractor Against The Ordering Party, And The Ordered Services As To The Number Of Hours Will
