The Subject Of The Order Is The Purchase, Delivery, Installation, Assembly And Commissioning Of A Comprehensive Ultrasound Machine, In Accordance With The Technical And Functional Parameters Specified In The File: Technical And Functional Parameters - Part 1, And In Accordance With The Indications Resulting From The Regulation Of The Minister Of Health Of September 24, 2013 On Guaranteed Services In The Field Of Primary Health Care. 2. The Subject Of The Order Also Includes Staff Training In The Use Of The Offered Medical Equipment. The Equipment Covered By The Subject Of The Order Will Be Delivered To: Independent Public Health Care Center Based In Szczerców, Ul. Mickiewicza 39B, 97-420 Szczerców; 3. The Ordering Party Requires That The Offered Equipment Covered By The Subject Of The Order: A) Be Brand New, Free From Any Physical Defects And Damage, Without Previous Use And Not Subject To The Rights Of Third Parties, B) Be Manufactured In 2026, C) The Device With Heads Is A Medical Device Approved For Trading And Use In Poland, In Accordance With The Act Of April 7, 2022 On Medical Devices, D) The Parameters Included In The Description Of The Subject Of The Order Are Absolutely Required. Failure Of The Offered Products To Meet Any Of The Required Parameters Will Result In Rejection Of The Offer As Inconsistent With The Content Of The Swz. 4. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Contract Template. 5. If The Description Of The Subject Of The Order Indicates Any Trademark, Patent Or Origin, Source Or Specific Process That Characterizes The Products Or Services Provided By A Specific Contractor - It Should Be Assumed That The Indicated Patents, Trademarks, Origin, Processes Determine The Technical, Operational And Operational Parameters, Which Means That The Ordering Party Allows The Submission Of An Offer In This Part Of The Subject Of The Contract With Equivalent Technical, Operational And Operational Parameters.
