The Purchase Is Carried Out As Part Of The Grant Project No. Fenx.06.01-Ip.03-0001/23 Support For Primary Health Care (Poz), For The Spzoz In Brzyska Application No. 09Ow/960/Ii/2026.3. The Material Scope Of The Grant Is A Closed List. The Contractor Cannot Offer An Item That Would Change The Type Of Item Indicated In The Project Schedule. Equivalent Products Meeting All Minimum Swz And Opz Requirements Are Permitted.4. The Equipment Must Be Brand New, Unused, Complete, Free From Physical And Legal Defects, Intended For Professional Use And Approved For Trading And Use In Poland In Accordance With Applicable Regulations.5. Medical Equipment Must Meet The Requirements Applicable To Medical Devices, Including The Requirements Of Regulation (Eu) 2017/745 (Mdr), If The Device Is Subject To This Regulation, And Have The Required Ce Marking And Documentation In Polish.6. The Comprehensive Ultrasound Device Must Also Meet The Requirements Applicable To Guaranteed Services In The Field Of Primary Health Care, In Accordance With The Regulations In Force On The Date Of Purchase.7. The Delivery Includes All Costs Of Transport, Unloading, Carrying In, Assembly, Commissioning, Testing, Staff Instruction And Transfer Of Documentation, If These Activities Are Required For A Given Item.8. Equipment And Supplies Purchased From The Grant Funds Will Be Located At The Place Where Services Are Provided Related To The Project - Health Center In Brzyska, Brzyska 349, 38-212 Brzyska - Except For The Cases Permitted By The Grant Documents.9. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 4 To The Swz.
