Part 1 - Speech Therapy Classes, Package I - Estimated Duration Of Up To 120 Hours;1. The Subject Of The Order Is The Provision Of Services In The Field Of Early Development Support For Children Under The Comprehensive Support Program For Families Za Życiem In 2026, Constituting An Annex To Resolution No. 189 Of The Council Of Ministers Of December 27, 2021 On The Comprehensive Support Program For Families Za Życiem (M.P. 2022, Item 64), Hereinafter Referred To As The Programme.2. The Services That Are The Subject Of This Order Will Be Provided In The Form Of Individual Classes With Children From Birth Until They Start School.3. One Hour Of Service Provision Is Considered To Be A Full Clock Hour (60 Minutes).4. The Ordering Party Emphasizes That The Term Hours Of Provision Of Services Should Be Understood Only As The Actual Time Of Providing Services To Or On Behalf Of The Child (This Time Does Not Include, For Example, The Time Necessary For The Person Providing Services To Move From One Child To Another, If The Person Provides Services To Several Children In One Day).6. Travel Costs Related To The Execution Of The Subject Of The Order Are Covered By The Contractor Within The Remuneration Specified In The Offer, Constituting The Maximum Obligation Of The Ordering Party.7. The Detailed Time And Place Of Providing Services Will Be Specified By The Ordering Party In The Schedule Of Classes Established Taking Into Account The Needs Of Participants Of Individual Therapies In Consultation With The Contractor.8. The Classes Will Be Carried Out At The Complex Of Educational And Upbringing Facilities In Gołdap, Address: Ul. Wojska Polskiego 18, 19-500 Gołdap.9. The Classes Will Be Carried Out In: A Therapeutic Room Equipped With Equipment And Aids Necessary To Conduct The Classes Covered By This Agreement.10. Classes Will Be Held From Monday To Friday From 3:00 P.M. To 6:00 P.M. And On Saturdays From 8:00 A.M. To 4:00 P.M. Nov. Classes Will Be Held In Accordance With The Number Of Hours Allocated To The Child, Determined By The Ordering Party According To The Agreed Schedule Of Classes.12.
