Tenders Are Invited For Purchase Of An Ambulance With Equipment Based On A Subsidy To Co-Finance The Implementation Of The Investment Project Entitled: Ambulance For The Hospital

The Subject Of The Order Is The Supply Of A Type C Or B Emergency Ambulance Adapted To Transport A Maximum Of One Patient On A Stretcher And A Three-Person Specialist/Basic Team Consisting Of: Driver, Paramedic, Doctor (Can Be Used As A Specialist (S) Or Basic (P) Ambulance In Accordance With The National Health Fund Guidelines). The Ambulance Must Meet The Requirements Of The Current Pn/En 1789 Standard For Type C Or B Ambulances Or Equivalent Standards.2. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1A To The Swz - Description Of The Subject Of The Order.3. The Subject And Conditions Of The Implementation Of This Order Should Be In Accordance With The Act Of April 7, 2022 On Medical Devices And Regulation (Eu) 2017/745 Of The European Parliament And Of The Council Of April 5, 2017 On Medical Devices Or Regulation (Eu) 2017/746 Of The European Parliament And Of The Council Of April 5, 2017 On In Vitro Diagnostic Medical Devices (If Applicable) And With Other Applicable Legal Provisions In This Regard. The Offered Vehicle And Equipment Must Have A Ce Declaration Of Conformity.4. The Subject Of The Order Is To Be Complete, Ready For Use Without Any Additional Purchases Or Investments. The Subject Of The Order Should Be Produced In 20265. The Offered Subject Of The Order Is To Be Brand New, Unused And Not Displayed At Exhibitions Or Fair Events, Not Being A Post-Demonstration Item, Not Previously Used For Any Purpose By Another Entity, Technically Efficient, Safe, Complete And Ready For Work, And Must Meet The Technical And Functional Requirements Specified In The Description Of The Subject Of The Order. The Ordering Party Does Not Envisage The Possibility Of Awarding Contracts Referred To In Art. 214 Section 1 Point 8 Of The Public Procurement Law.7. The Contractor Will Provide A Warranty For Defects And A Quality Guarantee For The Subject Of The Order: Min. 24 Months Without Mileage Limit - For The Base Vehicle (Warranty Period Is The Criterion For Evaluating Offers), Min. 60 Months From The Date Of Signing The Acceptance Report For Perforation Of The Ambulance Body. 24 Months From The Date Of Signing The Acceptance Protocol For The Installation Of The Ambulance. 24 Months From The Date Of Signing The Acceptance Protocol For Medical Equipment.
