Approximate Annual Demand For Fuel Supply: A) Diesel Oil (On) Approx. 37,000 Liters / 1 Year B) Unleaded Petrol (Pb95) Approx. 3,700 Liters / 1 Year C) Diesel Oil Additive (Adblue) Approx. 1,000 Liters / 1 Year 2. Fuels That Are The Subject Of The Order Must Meet The Standards Applicable In Poland In Accordance With The Regulation Of The Minister Of Economy Of October 9 2015 On Quality Requirements For Liquid Fuels (Journal Of Laws Of October 23, 2015, Item 1680).3. The Adblue Diesel Additive Must Meet The Technical Requirements In Accordance With Din 70070 And Iso 2241. The Ordering Party Allows The Sale Of Adblue In A Canister, E.G. 5L, 10L Or 20L.4. The Ordering Party Allows The Use Of Fuel Cards To Record Deliveries Under The Following Conditions: A) The Cards Will Be Issued Free Of Charge In The Quantity According To The List Provided By The Ordering Party, B) Only Fuels Which Are The Subject Of The Contract Can Be Purchased On The Card; C) The Card Will Be Assigned To A Specific Vehicle Or Equipment; D) The Card Will Record: - Date And Time Of The Transaction, - Type Of Fuel Purchased, - Quantity Of Fuel Purchased, - Address Of The Gas Station Where The Purchase Was Made. E) The Card Must Be Secured With A Code. Access And Enable Blocking In The Event Of Theft Or An Unauthorized User, F) The Supplier Will Provide Access To A Website Enabling Viewing And Printing Reports Of The Data Contained On The Cards. G) In The Event Of Loss Or Theft Of The Fuel Card, The Supplier, After Reporting This Fact By Telephone Or In Writing By The Ordering Party, Will Immediately Block The Fuel Card (The Notification Is Deemed To Have Been Made When A Call Was Made To The Telephone Number Indicated By The Supplier, Open 24 Hours A Day, 7 Days A Week, Or Written Information Was Sent By Fax Correctly). The Ordering Party Agrees That Reports Regarding The Loss Or Theft Of A Fuel Card Will Be Made Via The Contractors 24-Hour Hotline, Or The Ordering Party Will Block It On Its Own On The Available Fleet Management Internet Portal. No Later Than 14 Calendar Days From The Date Of Notification, J) In The Event Of Loss Or Theft Of A Fuel Card, With Each Card Issued The Ordering Party Will Pay A Fee In The Amount Applicable On The Date Of Issue Of The Fuel Card According To The Suppliers Tariff, K) In The Event Of Purchase Of A New Vehicle By The Ordering Party, A Fuel Card Will Be Issued For The New Vehicle At The Expense Of The Supplier, L) Fuel Cards Will Be Issued According To The List Provided By The Ordering Party, Specifying: The Number Of Cards, The Designation Of Cards For A Given Vehicle Or Equipment And The Type Of Fuel That Can Be Used For Purchase A Given Penalty.Ł) If It Is Not Possible To Register The Purchase Transaction On The Fuel Card (E.G.
