Medical Equipments – Endoscopy Laboratory Equipment. 1) The Subject Of The Order Is Equipment For The Endoscopy Laboratory. 2) The Subject Of The Order Is Described In Detail In Annex No. 1 To The Swz Product And Price Form (Description Of The Subject Of The Order - Opz). 3) Detailed Conditions And Rules Regarding The Implementation Of The Subject Of The Order Are Regulated In The Template Agreement, Annex No. 4 To The Swz (Contractual Conditions). 4) The Ordering Party Requests That The Contractor Indicate In The Offer The Parts Of The Order Whose Performance He Intends To Entrust To Subcontractors And That The Contractor Provide The Companies Of The Subcontractors - In Accordance With Art. 462 Section 2 Of The Public Procurement Law, If They Are Already Known. 5) If The Change Or Resignation From The Subcontractor Concerns The Entity Whose Resources The Contractor Relied On, In Accordance With The Principles Specified In Art. 118 Section 1 Of The Public Procurement Law, In Order To Demonstrate Compliance With The Conditions For Participation In The Procedure, The Contractor Is Obliged To Demonstrate To The Ordering Party That The Proposed Other Subcontractor Or Contractor Independently Meets Them To An Extent No Less Than The Subcontractor Whose Resources The Contractor Referred To During The Contract Award Procedure.
