The Device Must Have: All Elements Of The Offered Device Should Have Ce Certificates Of Conformity. The Contractor Will Offer The Device In Accordance With The Employers Requirements Specified In The Swz And The Procedure Documentation, And Is Obliged To Supplement The Table With Data Of The Offered Device Enabling Its Identification - List Of Devices, According To The Template Constituting Annex No. 2.0. To Swz - Offer Form - Indicating In Particular The Name Of The Manufacturer, Name Of The Device, Type/Model, Year Of Production And Other Important Parameters For Each Of The Elements Of The Device Constituting The Subject Of The Order And Their Price. Together With The Offer, In Order To Confirm That The Offered Supplies Meet The Requirements Specified By The Ordering Party And The Features Specified By The Ordering Party, The Contractor Is Obliged To Submit The Said Evidence, In Polish, In The Form Of: A) Catalog Cards, B) Manufacturers Technical Descriptions, C) Technical Specifications, D) Instructions, E) Manufacturers Documentation, F) List Of Parameters, G) Certificates Or Other Documents Confirming The Fulfillment Of The Requirements Specified In The Swz And Opz Regarding The Offered Devices, Listed By The Ordering Party In The Offer Form.
