The Subject Of The Order Is The Performance Of Demolition Works Of A Building Included In The Kowr Records As A Maternity Ward, Inventory No. 66 With Accompanying Infrastructure And Landscaping, The Facility Is Located On Plot No. 3/11, Wiatrowo Registration District, Damnica Commune, Słupsk Poviat, Pomeranian Voivodeship.2. The Detailed Scope Of Work Was Described In The Technical Description Of Demolition Works, Technical Specification For The Execution And Acceptance Of Construction Works, Bill Of Quantities, Valuation Of Recovered Materials And In The Draft Provisions Of The Contract.3. This Demolition Will Be Carried Out On The Basis Of The Demolition Notification And The Certificate Issued By The Staroste Of Słupsk On June 9, 2026, Stating That There Are No Contraindications To The Demolition Notification (Letter Number Ab.6743.382.2026.Viii).4. The Scope Of Demolition Works Includes: 1) Preparation By The Contractor, Before The Commencement Of The Works, Of An Ornithological Study In The Scope Of The Occurrence Of Birds Of Protected Species With A Written Declaration On The Location Or Lack Of Habitats Of Birds Of Protected Species (Applies To Construction Works Carried Out In The Period From March 1 To October 15), 2) Fencing And Marking Of The Demolition Area, 3) Complete Demolition Of The Facilities Together With Foundations And Other Elements Constituting Components And Functional Parts Located On The Surface And Under Surface, 4) Tidying Up And Cleaning The Construction Site From Rubble And Other Remains After Demolition And Removing Them In A Manner Consistent With Applicable Regulations, In Particular In Accordance With The Waste Act, 5) Planting To The Level Of The Surrounding Ground And Tidying Up The Construction Site, 6) Removing All Items Stored From The Demolished Facilities And Access Roads, Including Municipal, Bulky And Hazardous Waste, Garbage, Grass, Bushes And Other Items Resulting From The Technical Documentation And Technology Of Work Execution Demolition Works And Removing Them In A Manner Consistent With Applicable Regulations, In Particular The Regulations On Waste, 7) Performance Of Construction Works That Were Not Specified In The Bill Of Quantities And Technical Documentation But Are Necessary For The Implementation Of The Subject Of The Agreement, 8) Securing The Existing Trees And Securing Other Objects That May Be Damaged During Demolition Works, 9) Preparation By An Authorized Person And Delivery To The Ordering Party Of As-Built Geodetic And Cartographic Documentation For The Purpose Of Introducing Any Changes To The Register Of Land And Buildings, Along With Confirmation Of Submitting The Above Documents To The District Office.5.
