In The Case Of Using The Extended Transitional Period Due To The Need To Confirm That The Offered Medical Device Can Be Placed On The Market Under The Transitional Provisions, A Declaration On The Use Of The Transitional Period, The Manufacturers Own Declaration Specifying The Date Of End Of The Transitional Period, A Certificate Of The Manufacturers Quality Management System, Confirmation Of Concluding An Agreement On The Assessment Of The Conformity Of The Device With A Notified Body.
