1. The Subject Of The Order Is The Service Called Providing Comprehensive Nutrition Services For Patients At Pzol Grojec.2. The Description Of The Subject Of The Order Is Included In Annex No. 6 To Swz.3. The Ordering Party Reserves The Right Not To Complete The Entire Order If Its Actual Needs Are Lower Than Expected.4. Additional Requirements For The Entire Subject Of The Order: A) The Ordering Party Reserves The Right To Control The Supervision Of The Service Performance By Designated Persons, B) In Order To Enable The Inspection, The Contractor Undertakes To Present Appropriate Documents At The Ordering Partys Request And Provide All Necessary Information Regarding The Provision Of The Service, C) Any Comments Regarding The Quality Of The Service Are Reported By The Ordering Party To The Coordinator-Representative Of The Contractor After Preparing An Inspection Report, D) The Contractor Is Responsible For Any Irregularities Related To The Performance Of The Service.5. Requirements Regarding The Employment Of Persons By The Contractor Or Subcontractor On The Basis Of An Employment Relationship In Accordance With The Provisions Of Article 95(1) Of The Public Procurement Act: 1) Requirements Regarding The Employment Under An Employment Contract Of Persons Performing The Subject Of The Order - Type Of Activities To Which The Employment Requirements Apply: The Ordering Party Requires The Employment By The Contractor Or Subcontractor Under An Employment Contract Of Persons Performing The Following Activities: Preparation And Cooking Of Meals. The Ordering Party Allows An Employee To Be Employed On The Basis Of A Mandate Contract Only In Emergency And Short-Term Cases (E.G. Replacement, Illness, Other Random Events).2) No Later Than On The Day Of Signing The Contract And Upon Each Request Within A Period Specified By The Ordering Party, Not Shorter Than 5 Days, The Contractor Will Provide The Ordering Party With A List (Annex No. 7 To The Swz) Confirming: • The Requirement To Employ Persons Performing The Activities Indicated In Section 6 Point 1) On The Basis Of An Employment Contract Along With The Number Of These Persons, The Type Of Contract And The Working Hours. The List Should Specify The Entity On Behalf Of Which It Is Submitted (Contractor Or Subcontractor, Respectively) And Be Dated And Signed By The Person Authorized To Represent This Entity.3) During The Execution Of The Order, The Ordering Party Is Entitled To Carry Out Inspection Activities Towards The Contractor Regarding The Fulfillment By The Contractor Or Subcontractor Of The Employment Requirement Under The Employment Contract.
