The Subject Of The Order Is The Provision Of A Physiotherapy Interview Service And Stationary Physiotherapy Services In The Contractors Rooms/Rooms/Offices For Residents Of The Bielsk Subregion, Carried Out In Bielsk Podlaski And/Or In Another Place Indicated By The Ordering Party In The Bielsko Subregion. Services Will Be Provided To A Total Of 300 People (Possibly As Part Of An Option For An Additional 200 People) Who Need Physiotherapy Services In Order To Take Up Work Or Return To Employment. The Action Is Implemented Outside The Rpz Formula (Regional Health Program). The Order Includes Conducting A Physiotherapy Interview And Physiotherapy (Kinesitherapy And Massage) In The Nature Of Medical Rehabilitation In The Area Of ​​Physical Health Only In Connection With Diseases Of The Musculoskeletal System Or The Nervous System Or Circulatory System Or Cancer Diseases Or Chronic Diseases Of The Respiratory System. The Services Will Be Addressed To Project Participants - I.E. People Over 18 Years Of Age (Including Disabled People) Working In The Podlaskie Voivodeship, At Risk Of Leaving The Labor Market Due To Health Factors Or Unemployed People Registered As Unemployed In District Labor Offices In The Podlaskie Voivodeship, Who Need Physiotherapy Services In Order To Take Up Work Or Return To Employment, Who Have Been Qualified To Take Part In The Project And Have Signed Declaration Of Participation In The Project And Provided All Required Documents. The Age Of Participants Is Determined Based On Their Date Of Birth (Day, Month, Year) And Determined On The Day They Start Participating In The Project. The Ordering Party Is Responsible For Recruitment To The Service. The Contractor Should Provide Rooms/Rooms/Offices Adapted To The Needs Of People With Disabilities If A Given Service Participant Reports Needs In This Respect To The Contractor.
