Work Detail
1. The Subject Of The Order Is The Provision Of Services By A Special Educator, Psychologist, Speech Therapist, Physiotherapist And Sensory Integration Therapist As Part Of Specialized Care Services For People With Mental Disorders Referred To In Art. 3 Point 1 Letter A,B,C, Of The Act Of August 19, 1994 On The Protection Of Mental Health (Consolidated Text: Journal Of Laws Of 2024, Item 917), I.E. Persons: 1) Mentally Ill (Displaying Psychotic Disorders), 2) Mentally Disabled, 3) Showing Other Disturbances Of Mental Functions Which, According To The State Of Medical Knowledge, Are Classified As Mental Disorders, And These Persons Require Health Services Or Other Forms Of Necessary Assistance And Care To Life In A Family Or Social Environment. 2. Specialized Care Services For People With Mental Disorders, Including People With Autism And People With Intellectual Disabilities - Hereinafter Referred To As Services, Are Services Provided In The Place Of Residence And Adapted To The Specific Needs Resulting From The Type Of Their Disease Or Disability, Provided By People With Specialized Professional Training. 3. The Estimated Average Number Of Service Recipients Is Approximately 10 People. 4. Services Will Be Performed Depending On Needs, On Days And Hours Agreed Between The Contractor And The Service Recipient. 5. An Hour Of Service - 60 Minutes Includes Only The Actual Time Of Providing The Service, It Does Not Include The Therapists Journey To Or From The Patient. 6. The Estimated Number Of Hours Of Services During The Contract Period Is Approximately 255 Hours With The Following Reservations. 7. Due To The Specificity Of The Order, Which Is Characterized By Variability Of Needs Due To The Changing Health Condition And Life Situation Of People Requiring Help - Beneficiaries, The Ordering Party Reserves The Right To Limit The Subject Of The Order, However, The Minimum Scope Of Use Of The Estimated Number Of Hours Is Not Less Than 50%, Namely 127.5 Hours. The Contractor Will Not Be Entitled To A Claim For The Unused Number Of Hours. 8. The Contractor Is Obliged To Ensure The Ongoing Proper Provision Of Services By The Persons Subordinated To Him And To Self-Monitor The Correctness Of The Services Provided. For This Purpose, The Contractor Should Have An Internal Order Execution Control System. The Above Procedure Should Include Solutions Enabling The Contractor To Monitor The Services Provided On An Ongoing Basis.9. The Employers Employees Can Control The Quality Of Services Provided By The Contractor In Clients Homes And Assess Their Compliance With Administrative Decisions.10. The Contractor Should Cooperate With The Ordering Party On The Following Principles: • Ensure Constant Telephone Contact; • Guarantee Immediate On-Call Service In Emergency Cases; • Provide Specialist Services For People With Mental Disorders In Accordance With The Agreed Scope And Dimension; • If It Is Necessary To Change The Scope Of Specialist Care Services For People Wi
