The Subject Of The Proceedings And Order Is The Purchase Of 8 Pieces Of Hazardous Liquid Detectors Along With Delivery, Assembly, Commissioning, Performance Of Warranty Service Inspections Throughout The Warranty Period And Training Of The Ordering Partys Staff In The Following Locations: A) District Court In Łódź At Pl. Dąbrowskiego 5, B) District Court In Łódź At Ul. Narutowicza 64, C) District Court In Brzeziny At Ul. Waryńskiego 34, D) District Court In Brzeziny At Ul. Waryńskiego 32, E) District Court In Łowicz At Ul. Kaliska 1/3, F) District Court In Rawa Mazowiecka At Ul. Kościuszki 17, G) District Court In Rawa Mazowiecka At Ul. Zamkowa Wola 31, H) District Court In Łęczyca At Ul. M. Konopnicka 16. The Subject Of The Order Should Be Brand New, Unused, Not For Exhibition, Not Refurbished, Free From Construction, Material, Workmanship And Legal Defects, Manufactured No Earlier Than 2025, And Have The Name, Model, Manufacturer, Serial Number And Year Of Production Permanently Marked. The Terms And Conditions Of The Order Are Included In The Contract Template Constituting Annex No. 5.0. To Swz. The Contractor Will Deliver The Ordered Devices To The Users Address At Its Own Expense, Install And Start Them At The Workplace, And Train Users And People Responsible For The Proper Operation Of The Devices. The Device Must Have: All Elements Of The Offered Device Should Have Ce Certificates Of Conformity. The Contractor Will Offer The Device In Accordance With The Employers Requirements Specified In The Swz And The Procedure Documentation, And Is Obliged To Supplement The Table With Data Of The Offered Device Enabling Its Identification - List Of Devices, According To The Template Constituting Annex No. 2.0. To Swz - Offer Form - Indicating In Particular The Name Of The Manufacturer, Name Of The Device, Type/Model, Year Of Production And Other Important Parameters For Each Of The Elements Of The Device Constituting The Subject Of The Order And Their Price. Additionally, As Evidence, The Contractor Attaches: • A Certificate Regarding Industrial And Electromagnetic Safety (Emc). • Ecac Standard 3 Type B Safety Certificate. • Document Confirming Compliance With Health And Safety Requirements, Confirming Compliance With All Safety Requirements Contained In The Relevant Provisions And Standards Of Polish Law.
