The Subject Of The Order Is The Delivery Of: Delivery Of A Comprehensive Ultrasound Device, Including Doppler, With Cardio And Vascular Functions And With A Function For Examining The Abdominal Cavity For The Spgzoz In Zaleszany With/S In Zbydniów.2. A Detailed Description Of The Subject Of The Order/Form Confirming That The Offered Ultrasound Device Meets The Minimum Requirements Of The Ordering Party (List Of Technical And Operational Parameters, Required And Assessed) Is Attached As Annex No. 4 To Swz.3. The Subject Of The Order Also Includes Unloading, Carrying In, Assembly, First Start-Up And Training Of A Maximum Of 3 Employees Of The Ordering Party; Estimated Training Time: 1 Hour 4. The Offered Device Must Be Brand New, Manufactured No Later Than In 2026, Not For Exhibition, Not Reconditioned, Not Being A Demo Device - Complete And Ready For Use Without The Need To Incur Any Additional Investment Purchases. 5. The Offered Device Must Have The Required Certificates And Markings Provided For In The Provisions Of The Act On Medical Devices, Proving The Required Authorization For Use And Marketing In Poland, I.E. The Ce Certificate And Declaration Of Conformity In Accordance With The Regulation Of The Minister Of Health On Essential Requirements And Procedures For Assessing The Conformity Of Medical Devices. In Addition, The Offered Equipment Must Be A Medical Device Approved For Trade And Use In Poland In Accordance With The Provisions Of The Act Of April 7, 2022 On Medical Devices.6. The Contractor Is Obliged To Provide The Ordering Party With Operating, Maintenance And Servicing Instructions Prepared In Polish, As Well As Warranty Documents (Cards) And Other Necessary Documents (E.G. Technical Passports, Certificates) - Together With The Delivery Of The Device.7. For The Offered Subject Of The Order: (Camera, Accessories), The Contractor Will Provide At Least: 24-Month Warranty Period.8. The Order Is Implemented As Part Of The Fenx.06.01-Ip.03-0001/23 Project Called Support For Primary Health Care (Poz), Implemented Under The European Funds For Infrastructure, Climate, Environment 2021-2027 Program, Co-Financed By The European Regional Development Fund, Implemented Under Contract No. Fenx.06.01-Ip.03-0001/23-00/1374/2024/17 Concluded On March 27, 2024 Between The State Treasury - Minister Of Health And The National Health Fund.9.
