The Subject Of The Order Is The Delivery Of A New Type C Ambulance With A 4X4 Drive Along With Equipment In Accordance With Applicable Regulations And The Requirements Of The Ordering Party, Intended For The Implementation Of Tasks In The Field Of Civil Protection And Civil Defense. The Material And Quantitative Scope As Well As The Required Conditions For The Implementation Of The Subject Of The Order Covered By This Procedure Are Specified In: - Detailed Description Of The Subject Of The Order - Annex No. 1 To The Swz; - Draft Contract - Annex No. 4 To The Swz. Of This Specification Of Terms Of Order (Swz). The Content Of The Offer Form Included In Annex No. 2 To The Swz And The Price Form Included In Annex No. 3 To The Swz Should Clearly Indicate In The Indicated Scopes What Product The Contractor Offers, By Specifying Data Allowing Identification Of The Offered Product Range, I.E. At Least The Trade Name/Model, Assigned Catalog Number (If Marked), Manufacturers Name, Etc. Data Identifying The Offered Product Range Should Be Identical To The Data Provided By The Contractor. Will Be Used When Issuing An Invoice. The Subject Of The Order Should Be Brand New, Free From Physical And Legal Defects, Cannot Be Encumbered With The Rights Of Third Parties And Cannot Be The Subject Of Any Proceedings (Including Administrative, Tax, Criminal) Or Security. The Ordering Party Requires (If Applicable) That The Offered Service, Including All Ict Products, Ict Services And Ict Processes Used To Perform The Order, Both As Main Elements And Components, Elements Of The Service Provision Environment Or Accompanying Solutions, Does Not Include: • Ict Products, Ict Services Or Ict Processes Indicated In The Recommendation Referred To In Art. 33 Section 4 Of The Act Of 5 July 2018 On The National Cybersecurity System, Stating Their Negative Impact On The Fundamental Interest Of State Security; • An Ict Product Whose Type Was Specified In The Decision To Recognize The Supplier As A High-Risk Supplier Referred To In Art. 67B Section 15 Of The Act Of 5 July 2018 On The National Cybersecurity System, Nor The Ict Services Or Ict Processes Specified In This Decision. Failure To Meet The Above Requirement Results In Rejection Of The Offer Pursuant To Art. 226 Section 1 Point 17 Or Point 19 Of The Public Procurement Law.
