Test Tubes – Delivery Of Consumables For Testing For Inlife Iii. 1. The Subject Of The Order Is The Supply Of Consumable Materials For Scientific Research Conducted At The Inlife Institute Of Animal Reproduction And Food Research Of The Polish Academy Of Sciences. A Detailed Description Of The Subject Of The Order Can Be Found In The Offer Form - Annex No. 1 To The Swz. The Ordering Party Did Not Allow The Submission Of Partial Offers In The Procedure Due To The Uniform Subject Of The Contract. Each Contractor May Submit One Offer. 2. The Offered Consumable Materials And Laboratory Accessories Must Meet The Requirements In Terms Of Dimensions, Type Of Material, And Additional Parameters Described In The Offer Form - Annex No. 1 To The Swz. The Laboratory Materials Offered Must Be Approved For Marketing In The Republic Of Poland Or The Eu. 3. The Contractor Delivers The Products With Attached Safety Data Sheets And/Or Quality Specifications Containing Basic Data And Physical And Chemical Requirements Confirming Compliance Of The Delivered Products With The Swz. 4. The Warranty Period (Expiration Date) For Products Cannot Be Shorter Than 12 Months From The Date Of Delivery. The Procedure For Checking And Confirming The Compliance Of The Delivered Order And The Method Of Reporting Complaints In Accordance With The Provisions Of The Draft Provisions Of The Contract - Annex No. 3 To The Swz. 5. The Contract With The Contractor Will Be Concluded For A Period Of 12 Months From The Date Of Signing. The Partial Delivery Date Will Be Consistent With The Date Offered By The Contractor In The Offer: Up To 30 Days From The Date Of Placing The Order By The Ordering Party. Delivery Date Is One Of The Criteria For Evaluating Offers. 6. Subject Means Of Evidence A. The Ordering Party Requires The Contractor, Along With The Offer, To Submit Cards Of The Offered Materials And/Or A Quality Specification Containing Basic Data And Physical And Chemical Requirements Confirming That The Offered Products Meet All The Ordering Partys Requirements Indicated In The Description Of The Subject Of The Order. B. The Contractor Submits Safety Data Sheets With The Offer. The Contractor Submits The Documents In The Original Or A Copy Certified As True To The Original In Electronic Form With A Qualified Electronic Signature. If The Original Document Referred To Above Was Not Prepared In The Form Of An Electronic Document, The Contractor May Prepare And Submit An Electronic Copy Of The Document In Its Possession. If The Contractor Provides An Electronic Copy Of The Document, Affixing It With A Qualified Electronic Signature Is Tantamount To Certifying The Electronic Copy Of The Document As Consistent With The Original. Documents Prepared In A Foreign Language Must Be Submitted With A Translation Into Polish. C. If The Contractor Does Not Submit The Above-Mentioned Evidence Or They Are Incomplete, The Ordering Party, In Accordance With Art. 107 Section 2 Of The Public Procurement Law, Will Call On The Contractor Once Within The Specified Deadline To Submit Or Supplement Them.
