1. The Subject Of The Order Is: Collection And Management Of Waste From The Selective Municipal Waste Collection Point (Pszok) Of The City Of Otwock, Located At Ul. Kraszewskiego 1, 05-400 Otwock, For A Period Of: 12 Months From The Date Of Signing The Contract.2. Description Of The Subject Of The Order: 1. Amounts Of Waste And Type Of Waste For Each Part Of The Order - In Accordance With The Annex Called: Pszok - List Of Waste.2. The Above List Should Be Treated As An Estimate. The Contractor Is Obliged To Collect Any Amount Of Waste From The Pszok In Question.3. The Contractor Is Obliged To Collect The Waste Within Two Days From The Date Of Being Informed About It By The Ordering Party. The Ordering Party Will Indicate In The Notification The Amount And Fractions Of The Given Waste.4. Reports And Statementsa. The Contractor Prepares A Report Informing About The Contractors Fulfillment Of The Subject Matter Of The Contract, In Electronic Or Written Form Agreed With The Ordering Party. B. The Contractor Is Obliged To Deliver The Report To The Ordering Party Within 14 Days From The End Of The Period To Which It Relates.C.
