Part No. 1 Of The Order Entitled Transport Services For Transporting Disabled Students Residing In The Miedźno Commune, Including Providing Care During Transport In The 2026/2027 School Year - To And From The Special Educational And Upbringing Center In Działoszyn1. The Subject Of Part No. 1 Of The Order Is The Execution Of The Order Titled Transport Services In The Field Of Transport Of Disabled Students Residing In The Miedźno Commune, Including The Provision Of Care During Transport In The School Year 2026/2027 - To And From The Special Educational And Educational Center In Działoszyn Including The Transport Of 2 Disabled Students (This Is An Estimated Number That May Change) From The Miedźno Commune To The Special Educational And Educational Center In Działoszyn Educational Center In Działoszyn And Back In The School Year 2026/2027 (01/09/2026 - 25/06/2027 - 187 Days) Along With Providing Care During Transport: A) Route: Miedźno-Kołaczkowice-Działoszyn (Special Educational Center In Działoszyn, 98-355 Działoszyn, Ul. Grota Roweckiego 2), B) Estimated Number Of Kilometers Per Day: 50 Km, C) Estimated Number Of Students Transported: 2, Including 1 In A Wheelchair. The Transport Service Must Be Provided By Means Of Transport Adapted To Transport Disabled Students, Meeting All Safety Requirements And The Road Traffic Code, Equipped With Certified Equipment Securing The Transport Of Persons And Enabling The Transport Of Wheelchairs, Equipped With Seat Belts, Ramp/Ramp Or Ramp Rails, As Well As A System That Prevents The Wheelchair From Moving In The Vehicle. The Driver Performing The Order Must Have Appropriate Authorizations To Provide The Service And At Least 3 Years Of Experience. In A Situation Where Transport Along The Route Is Not Possible (E.G. Renovation), The Transport Should Be Carried Out As Close To The Basic Route As Possible. After Removing The Reason For The Route Closure, Transport Should Return To The Basic Route. In The Event Of A Failure Of The Means Of Transport, The Contractor Will Introduce A Replacement Means Of Transport That Will Meet The Conditions Contained In The Description Of The Subject Of The Order. Transport Services Do Not Include Access To The Route And Technical Journeys. During The Service, The Route May Be Modified Depending On The Needs Of The Ordering Party. The Contractor Is Fully Responsible For The Safety Of Transported Students And Must Have Carriers Civil Liability Insurance For The Services Provided, In Particular Full Liability Insurance For Passengers. The Contractor Employs A Guardian. The Caregiver Will Be Equipped With A Reflective Vest To Increase Safety. Only An Adult Person Who Has Knowledge In The Field Of Providing Medical Or Pre-Medical First Aid And Is Prepared To Perform Basic Nursing And Care Activities For Disabled Students, Who Is Not Included In The Register With Limited Access Or In The Register Of Persons In Relation To Whom The State Commission For Counteracting The Sexual Abuse Of
