An Ict Product Whose Type Was Specified In The Decision To Recognize The Supplier As A High-Risk Supplier Referred To In Art. 67B Section 15 Of The Act Of 5 July 2018 On The National Cybersecurity System, Nor The Ict Services Or Ict Processes Specified In This Decision. The Contractor Submits, Together With The Offer Form (Appendix No. 2) To The Swz, List Of Ict Elements Used During The Execution Of The Order, Which Constitutes Appendix No. 2 A) To The Fo. This List Is An Integral Part Of The Offer And Serves To Identify Ict Products, Ict Services And Ict Processes Covered By The Offer Or Used To Complete The Order. The List Constitutes The Subject Meas Of Evidence. In Order To Confirm Compliance Of The Offered Subject Of The Order With The Requirements Specified In Points 4.22.1 And 4.22.2 Above, The Contractor Submits The Evidence Together With The Offer, I.E. A Declaration, The Template Of Which Is Annex No. 2 A) To The Fo, List Of Ict Elements Used During The Execution Of The Order. Contractor In The Above-Mentioned The List Is Obliged To Clearly Identify Each Ict Solution Offered, In Particular By Indicating: - Name Of The Ict Solution - Ict Product / Ict Service / Ict Process, - Manufacturer / Supplier / Service Provider Of The Ict Solution, - Type / Model / Version / Variant / Or Other Marking Allowing Identification Of The Ict Solution, - Role / Function Of The Ict Solution Performed As Part Of The Order Execution, - Notes, Other Information - If Applicable. 4.3. If The Content Of The Offer, Declaration, Documents Submitted With The Offer Or The Contractors Explanations Indicate That The Offer Includes The Solution Referred To In Section 4.22. Point 4.22.1 Or 4.22.2, The Offer Is Subject To Rejection Pursuant To Art. 226 Section 1 Point 17 Or Point 19 Of The Public Procurement Law. The Ordering Party Accepts Equivalent Means Of Evidence If They Confirm That The Offered Supplies And Services Meet The Requirements, Features Or Criteria Specified By The Ordering Party. If The Contractor Does Not Submit The Evidence In Question Or The Evidence Submitted Is Incomplete, The Ordering Party Calls For Its Submission Or Completion Within The Specified Deadline. The Above Provision Does Not Apply If, Despite The Submission Of The Evidence In Question, The Offer Is Rejected Or There Are Grounds For Invalidation Of The Proceedings.
