Health And Social Work Services – Jobcenter Mannheim - Health Center Project. The Client Intends To Outsource The Implementation Of Individual Health Care Measures For Unemployed People Entitled To Benefits In Accordance With Section 16 Paragraph 1 Sgb Ii In Conjunction With Section 45 Paragraph 1 Sentence 1 No. 1 Sgb Iii In The Form Of Operating A Health Center In Mannheim. The Target Group Are Those Entitled To Benefits In The Legal Circle Of Sgb Ii Who Have Generally Reached The Age Of 25 And Who Have Health - Physical Or Psychological - Impairments Or Limitations. The Contractor Must Maintain Places For 40 Participants Per Month Throughout The Life Of The Health Center. The Client Assigns The Participants To The Health Center For A Period Of Four (4) Weeks. With An Initial Term Of The Health Center Of 12 Months, An Allocation Period Of 4 Weeks, The Possibility Of Weekly Entry, And 40 Places For Participants Per Month, This Means A Maximum Number Of 480 Participants Per Year. The Measure Is Aimed At Maintaining, Restoring And Improving The Earning Capacity Of Those Entitled To Benefits. The Individual Objectives Of The Measure Are Listed In The Service Description (Appendix 4 Of The Tender Documents). In Order To Achieve These Goals, The Health Center Measure Provides For The Following Content (Measure Content): 1) Medical-Professional Assessment/Coaching/Abc Coaching And Needs Assessment (See Section Vi.3 Of The Service Description) 2) Reducing Health-Related Restrictions On Participation (See Section Vi.4 Of The Service Description) 3) Promoting Social And Cultural Participation (See Section Vi.5 Of The Service Description). With Regard To The Details Of The Service Owed, Reference Is Made To The Tender Documents, In Particular The Service Description (Appendix 4 Of The Tender Documents).
